摘要
目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的体重指数(BMI)与白天嗜睡及呼吸功能紊乱的关系。方法经多导睡眠图(PSG)确诊为OSAHS的患者114例,按BMI<24kg/m2(正常)、24kg/m2≤BMI<28kg/m2(超重)及BMI≥28kg/m2(肥胖)分为3组。采用Epworth嗜睡量表(ESS)及多导睡眠图测量其白天嗜睡状况及呼吸功能指标(如:呼吸暂停低通气指数(AHI)、夜间平均氧饱和度(MSaO2)、最低氧饱和度(LSaO2)、氧饱和度<90%时间占睡眠时间的百分比(Sa90))。结果肥胖组的ESS、AHI、Sa90均显著高于超重组和正常组,而MSaO2、LSaO2显著低于这两组(P<0.05)。相关分析结果显示:BMI与ESS、AHI、Sa90呈显著正相关(P<0.01),与MSaO2、LSaO2呈显著负相关(P<0.01)。结论肥胖对OSAHS患者的白天嗜睡症状及呼吸功能均有影响。
Objective:To explore the relationship between daytime sleepiness ,respiratory dysfunction and body mass index (BMI) in patients with obstructive sleep apnea hypopnea syndrome(OSAHS). Methods: 114 patients with OSAHS were diagnosed by polysomnogram(PSG) and grouped according to BMI. The status of sleepiness were tested by the Epworth Sleepiness Scale (ESS). The.respiratoIy parameters ( such as apnea hypopnea index (AHI) ,mean Oxygen Saturation (MSaO2 ) , Lowest Oxygen Saturation (LSaO2 ) , the percent of the total time with oxygen saturation level 〈 90% (Sa90)were tested by PSG). Results:Tile mean global score of ESS, AHI and Sa90 are significantly higher in obeses group than the overweigth and normal group. The MSaO2 , LSaO2 are significantly lower in obeses group than the overweight and normal group(P 〈 0.05 ). There were significantly correlation between ESS, AHI, MSaO2, LSaO2 ,Sa90 and BMI (P 〈 0.01 ). Conclusion:BMI may affect the daytime sleepiness, respiratory function and arterial oxygen saturation in patients with OSAHS.
