摘要
目的:研究雌激素对绝经后女性冠心病患者血脂及血管内皮功能的影响,探讨绝经后女性冠心病患者血脂及血管内皮功能的相关性。方法:将60例绝经后女性冠心病患者随机分为治疗组和对照组各30例,治疗组在常规治疗的基础上服用尼尔雌醇2 mg,2周1次,对照组仅采用常规治疗,均连续服用6个月。于治疗前及治疗后检测血清总胆固醇(TC)、血清三酰甘油(TG)水平,低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)以及血浆内皮素(ET-1)和一氧化氮(NO)水平,并观察疗效。结果:①与治疗前相比,治疗组TC、TG、LDL-C、ET-1水平均明显下降,HDL-C、NO水平明显上升;②与对照组治疗后相比,治疗组TC、TG、ET-1水平均明显下降,HDL-C、NO水平明显上升,差异均有统计学意义(P<0.05);③Pearson相关分析显示,ET-1与TC、TG及LDL-C之间呈正相关,与HDL-C呈负相关;NO与TC、TG及LDL-C之间呈负相关,与HDL-C呈正相关,差异均有统计学意义(P<0.05)。结论:血脂紊乱及血管内皮功能异常可能参与了绝经后女性冠心病的发生和发展,二者具有相关性;雌激素对绝经后冠心病患者的血脂有明显改善,并具有保护血管内皮功能的作用,有利于冠心病的防治。
Objective: To study the effect of estrogen on blood lipid and vascular endothelial function of postmenopausal women with coronary heart disease, explore the correlation between blood lipid and vascular endothelial function in postmenopausal women with coronary heart disease. Methods: 60 postmenopausal women with coronary heart disease were divided into treatment group and control group, 30 women in each group, the women in treatment group were treated with oral administration of nilestriol (2 mg) biweekly on the basis of routine treatment, while the women in control group were treated with routine treatment, all the women in the two groups were treated for 6 months. The serum levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterin (LDL -C), high density lipoprotein cholesterin ( HDL - C), endothelin - 1 and nitric oxide in the two groups were detected before and after treatment, the clinical efficacies in the two groups were observed. Results : Compared with before treatment, the serum levels of TC, TG, LDL - C and endothelin - 1 in treatment group decreased significantly after treatment, the serum levels of HDL - C and nitric oxide increased significantly ; compared with control group, the serum levels of TC, TG and endothelin - 1 in treatment group decreased significantly, the serum levels of HDL - C and nitric ox- ide increased significantly , there was significant difference (P 〈 0. 05 ) . Pearson correlation analysis showed that there was positive correlation between endothelin - 1 and TC, TG, LDL - C, and there was negative correlation between endothelin - 1 and HDL - C ; nitric oxide was negatively correlated with TC , TG and LDL - C, while nitric oxide was positively correlated with HDL - C, there was significant difference (P 〈 0. 05) . Conclusion: Dyslipidemia and vascular endothelial dysfunction may participate in the occurrence and development of coronary heart disease in postmenopausal women, there is correlation between blood lipid and vascular endothelial function; estrogen can improve blood lipid and protect vascular endothelial function of postmenopausal women with coronary heart disease, which is helpful to prevent and treat coronary heart disease.
