ROC曲线评价高岭土在大鼠化疗性呕吐模型中应用价值 被引量:4

摘要 目的探讨高岭土摄入量在大鼠化疗性异食癖模型中对呕吐反应筛检的应用价值。方法 45只雄性SD大鼠随机分为3组:空白对照组、模型组及昂丹司琼组,以6 mg/kg腹腔注射顺铂制造化疗性大鼠异食癖恶心呕吐模型,采用Biopac专用胃肠电记录分析系统同步检测胃体和胃窦部平滑肌电活动,依据胃电节律变化特征,运用ROC曲线评价高岭土在大鼠化疗性呕吐模型中的应用价值。结果造模后24 h,模型组大鼠高岭土摄入量显著高于空白对照组(P<0.01),且模型组大鼠胃肌电逆行扩布发生率显著高于空白对照组与昂丹司琼组(P<0.05);ROC曲线分析显示,高岭土摄入量在0.65 g时Youden指数最大(57.4%),其中灵敏度为72.2%,特异度为85.2%,ROC曲线下面积为0.827(95%CI:0.704~0.950,P<0.001)。结论大鼠化疗后高岭土异食癖行为可用作有呕吐反射动物的呕吐模型,高岭土摄入量在0.65 g及以上可能为化疗性大鼠呕吐反应筛检的理想参考值。 Objective To study of utilizing value of Kaolin intake for chemotherapy-induced vomiting assessed with ROC curve in rat. Methods Forty-five male SD rats were randomized into control group, model group and ondansetron group. Rats were injected intraperitoneally with cisplatin 6 mg/kg to induce pica according to method of De Jonghe. Using biological function of experimental system to record and analyze electrical activity of corpus and antrum gastrieum in rat, and based on gastric electrical rhythmic character, to study of utilizing value of Kaolin intake for chemotherapy-induced vomiting assessed with ROC curve in rat. Results 24 h after cisplatin administed, Kao- lin intake in the model group was significantly higher than those in the control group ( P 〈 0. 01 ), and incidence rate of gastric electrical retrogression transmission in the model group was also higher than that in the control group and ondansetron group (P 〈 0.05 ), respectively. The results of ROC curve showed that, according to the principle of the highest value of Youden index ( 57.4% ), the key point va)ue of Kaolin intake using to screening chemotherapy-induced vomiting was 0. 65 g, the sensitivity and specificity was 72.2% , 85.2% , respectively, and the area under ROC curve was 0. 827 (95% CI: 0. 704 -0. 950, P 〈 0. 001 ). Conclusion Chemotherapy-induced pica in rat can be used as the vomiting model, the reference value of Kaolin intake for screening vomiting induced by cisplatin is 0.65 g.
