广州地区急性呼吸道感染患者的病毒检出情况 被引量:13

摘要 【目的】了解广州医学院第二附属医院急性呼吸道感染就诊人群病毒流行病学特征。【方法】采用多重RT-PCR法,对2006.9至2009.8广州医学院第二附属医院门诊急性呼吸道感染患者鼻咽拭子检测呼吸道合胞病毒(RSV)、鼻病毒(HRV)、流感病毒A型(FluA)、流感病毒B型(FluB)、人偏肺病毒(HMPV)及腺病毒(ADV)、副流感病毒(PIV)、冠状病毒(HCoV)8种病毒,分析其年份、季节及年龄分布特点。【结果】3年共收集1388例标本,病毒检出率40.634%,RSV毒检出率最高(17.8%),其检出率逐年下降(P=0.000);HMPV检出率于第2年升高(P=0.016),第3年下降(P=0.000)。RSV春季检出率高(P=0.009);HRV秋季检出率高(P=0.001);PIV春季检出率高(P=0.000),夏季(P=0.023)、冬季(P=0.048)检出率低;FluA夏季检出率高(P=0.000);HMPV春季(P=0.05)、秋季(P=0.01)检出率高。各季节不同病毒检出率有差异:春季,RSV(P=0.000)及HRV(P=0.004)检出率较高;夏季,HRV(P=0.000)及FluA(P=0.000)检出率较高;秋季,HRV(P=0.000)检出率较高;冬季,HRV(P=0.002)及RSV(P=0.000)检出率较高。各年龄段病毒分布有差异:RSV(P=0.004)、HRV(P=0.029)及HMPV(P=0.001)于<3岁组检出率较高;ADV(P=0.000)与Flu B(P=0.011)于3~<14岁组检出率较高;Flu A(P=0.011)于14~<40岁组检出率较高。【结论】病毒有季节流行趋势,RSV春季流行,FluA夏季流行,HRV则全年高发;病毒检出率与年龄有相关性,RSV、HRV及HMPV检出多见于婴幼儿,ADV与Flu B多见于3~<14岁人群,Flu A于14~<40岁组检出率较高。RSV检出率最高,并有年份强势流行趋势;且是各年龄组急性上呼吸道感染的主要病毒之一。 【Objective】 To study the epidemiology of respiratory syncytial virus(RSV),human rhinovirus(HRV),influenza virus type A(flu A),influenza viruses type B(flu B),human metapneumovirus(Hmpv),adenovirus(ADV),parainfluenza virus(PIV) and human coronaviridae(HCoV) in patient with acute respiratory infection in Guangzhou.【Methods】 By multiplex RT-PCR,the above 8 virus were detected in the nasopharyngeal swabs of patient with acute respiratory infection who came to the clinic of the number two hospital of Guangzhou medical college during September,2006 to August,2009.And analysis was made to the distribution of season,year and age.【Results】 1388 nasopharyngeal swabs were collected in the 3 years,and the positive rate of virus detection was(40.634%),with RSV the most common virus detected(17.8%).The rate of RSV detected decreased each year(P = 0.000),while HMPV increased in the second year(P = 0.016).There was some difference on the distributing of time of the virus.The infection rate of RSV was high in 2007,and it increased in spring over the 3 years(R = 25.2%,P = 0.001).The infection rate of HRV increased in autumn(P = 0.001).The infection rate of PIV increased in spring(P = 0.000),and decreased in summer(P = 0.023) and winter(P = 0.048).The infection rate of flu A increased in summer(P = 0.000).The infection rate of HMPV increased in spring(P = 0.05) and autumn(P = 0.01).And there was also some difference on the infection rate between different viruses in the same season.The infection rate of RSV(P = 0.000) and HRV(P = 0.004) were higher than other virus in spring.In summer,the infection rate of HRV(P = 0.000) and flu A(P = 0.000) were higher.In autumn,the infection rate of HRV was higher(P = 0.000).And in winter,the infection rate of HRV(P = 0.002) and RSV(P = 0.000) were higher.The infection rates of virus differ from different population.The infection rate of RSV(P = 0.004),HRV(P = 0.029) and HMPV(P = 0.001) increased in children.The infection rate of ADV(P = 0.000) and Flu B(P = 0.011) increased in population of age of 3 ~ 14.The infection rate of Flu A increased in population of age of 14 ~ 40(P = 0.011).【Conclusion】 RSV was popular in spring,Flu A was popular in summer,and the infection rate of HRV was high through the whole year.RSV,HRV,and HMPV was popular in infant,ADV and Flu B were popular in population of age of 3 ~ 14,and Flu A was popular in population of age of 14 ~ 40.RSV was one of the most common virus detected,with its infection rate of high in 2007.
