摘要
目的探讨重型颅脑损伤患者早期经皮内镜下胃造口(percutaneous endoscopic gastrostomy,PEG)置管肠内营养的可行性及疗效。方法 2010年4月至2011年5月对32例重型颅脑损伤患者采用早期经皮PEG置管行肠内营养治疗。结果 32例患者均在颅脑损伤或开颅手术后第3~7天内实施PEG;全部置管成功,无操作死亡病例。并发症包括造口区皮肤渗血2例,红肿5例,感染1例,吸入性肺炎2例,上消化道出血1例。胃造口营养管保留时间7~185天,平均31.6天;32例中成活27例,死亡5例。结论重型颅脑损伤患者早期经皮PEG置管肠内营养有利于减少并发症,改善生存质量,且操作安全、舒适,值得推广应用。
Objective To investigate the feasibility and efficacy of enteral nutrition by early intubation through percutaneous endoscopic gastrostomy(PEG) for severe traumatic brain injury.Methods Thirty-two patients with severe traumatic brain injury were treated with enteral nutrition therapy by early intubation through PEG from Apr 2010 to May 2011.Results All patients underwent PEG 3~7 days after head injury or craniotomy.Intubation was successfully carried out in all cases with no deaths during the operation.The complications included 2 cases of errhysis in stoma area,5 cases of red swelling of the skin,1 case if infection,2 cases of aspiration pneumonia,and 1 case of upper gastrointestinal bleeding.Gastrostomy feeding tube was retained for 7~185 days(an average of 31.6 days).Twenty-seven cases survived,and 5 died.Conclusion Severe head injury patients treated with enteral nutrition by early intubation through PEG will help reduce complications and improve quality of life,and is safe and comfortable,which should be widely applied.
