摘要
目的探讨联合超滤技术(零平衡超滤、常规超滤、改良超滤)在心脏及大血管破裂急救中的应用。方法回顾性分析在急诊抢救心脏及大血管破裂患者的手术中运用联合超滤技术的相关资料。在抢救过程中,装机时按改良超滤的方法连接;转流开始即开放超滤端行零平衡超滤和常规超滤;停机后行改良超滤。结果 2例心脏破裂患者抢救成功。其中1例患者术后出现间歇性出昏迷、烦躁、嗜睡,查体见神志恍惚,反应迟钝,10 d后症状缓解;另1例术后无任何并发症。结论联合超滤的时间和超滤液量与患者的预后密切相关。
Objective To explore the application of the combined ultrafiltration technique(zero-balanced ultrafiltration,conventional ultrafiltration,modified ultrafiltration) in the first aid treatment for heart and great vessels rupture.Methods The emergency rescues of cardiac and large blood vessel rupture using combined ultrafiltration technology in our department were retrospectively analyzed.In the rescue process,modified ultrafiltration connection was installed.Zero-balance ultrafiltration and conventional ultrafiltration were opened as soon as the beginning of bypass.Modified ultrafiltration was carried out once the machine was stopped.Results Two cases of cardiac rupture were rescued,including one case without postoperative complication and one case with intermittent coma,irritability,lethargy,trance and unresponsiveness postoperatively(the symptoms disappeared after 10 days).Conclusion The prognosis of cardiac rupture is closely correlated with the time and volume of combined ultrafiltrate.
