64排螺旋CT灌注成像对良性前列腺增生的诊断价值研究 被引量:1

摘要 目的探讨64排螺旋CT灌注成像对良性前列腺增生(benign prostatic hyperplasic,BPH)的诊断价值。方法选择2006年8月-2010年5月张家口市第一医院收治的BPH 80例,另选择正常前列腺15例为对照组。各组均行64排螺旋CT灌注成像技术检查,对比各组灌注参数之间的差异。结果 BPH的TDC曲线为缓升型。BPH组的BF、PS、TTP明显高于对照组(P<0.05),MTT、TTP值明显小于对照组(P<0.05)。各级别BPH均呈现均匀灌注,四个灌注参数比较除BV外,BF、PS、MTT、TTP差异均无统计学意义(P>0.05)。BPH各级别间两两比较,BF、PS、MTT、TTP差异均无统计学意义(P>0.05),而BPHⅡ和BPHⅢ的BV值均明显大于BPHI组,BPHⅢ大于BPHⅡ组,差异均有统计学意义(P<0.05)。结论 64排螺旋CT灌注成像BV值的升高对BPH具有重要的诊断价值,但灌注成像无法对BPH进行分级。 Objective To explore the diagnostic value of 64-multidetector spiral computed tomography(64-MSCT) perfusion imaging in benign prostatic hyperplasic(BPH). Methods Eighty patients with BPH who were treated in the First Hospital of Zhangjiakou from August 2006 to May 2010 were enrolled in the study,and 15 healthy individuals were served as control group.64-MSCT perfusion imaging was performed in 80 patients and 15 healthy individuals.And the differences of perfusion parameters,including mean blood flow(BF),blood volume(BV),time to peak(TTP), mean transit time(MTT),capillary permeability-surface(PS),and time-density curve(TDC) in the two groups were analyzed and compared. Results The variation of TDC rose slowly in BPH group.BF,PS and TTP of BPH group were significantly higher than those in control group(P〈0.05),while TTP and MTT of BPH group were significantly less than those in control group(P〈0.05). No statistically significant difference was found in BF,PS,MTT and TTP among BPH I,BPH Ⅱ,and BPH Ⅲ(P〈0.05) except BV.BVs of BPH Ⅱ,and BPH Ⅲ were significantly higher than that of BPH I,while BV of BPH Ⅲ was higher than that of BPH Ⅱ,and the differences were statistically significant(P〈0.05). Conclusions Through performing 64-MSCT perfusion imaging,the increased BV shows a significant value in diagnosis of BPH,but CT perfusion imaging is unable to classify BPH.
