186株鲍曼不动杆菌临床分布及耐药性分析 被引量:5

摘要 目的调查鲍曼不动杆菌的临床分布及其对抗菌药物的耐药情况,为临床合理用药提供依据。方法将我院临床各种来源的鲍曼不动杆菌186株采用K-B法进行药敏试验,并对结果进行统计分析。结果 186株鲍曼不动杆菌临床标本中174株来源于痰液,其他来源依次为伤口拭子6株,尿液4株,其他标本2株;分布在重症监护病房(ICU)67株、呼吸科63株,神经内科22株,神经外科8株,干部病房及消化科各7株,烧伤科及其他科各6株。药敏试验结果显示鲍曼不动杆菌对目前推荐的多数抗菌药物的耐药率在50%以上,多重耐药菌株和泛耐药菌株分别占43.0%和22.6%6,1.25%的多重耐药株对推荐使用的头孢菌素类、碳青霉烯类、哌拉西林-他唑巴坦和氟喹诺酮类同时耐药。结论我院鲍曼不动杆菌耐药情况相对严重,临床必须根据药敏试验结果合理用药,并且有必要采取有力措施控制多重耐药株和泛耐药株的产生与传播。 Objective To investigate the clinical distribution of Acinetobacter baumannii and their resistance to antibacterials for rational use of antibiotics in clinical therapy.Methods Bacterial susceptibility testing was performed in 186 strains of Acinetobacter baumannii isolates from different clinical departments,and the results were analyzed.Results Among 186 strains of Acinetobacter baumannii isolates,174 were obtained from sputum samples,6 from wound swabs,4 from urine,and 2 from other samples;67 from ICU,63 from Pneumology Department,22 from Neurology Department,8 from Neurosurgery Department,7 from Cadre's Wards and Digestive System Disease Department,and 6 from Burn Department and other departments.Resistance rates of Acinetobacter baumannii to most introduced antibiotics were above 50%,multidrug-resistant and pandrug-resistant strains accounted for 43.0% and 22.6% respectively,and 61.25% of multidrug-resistant strains were resistant to introduced carbapenems,cephalosporins,fluoroquinolones and piperacillin/tazobactam simultaneously.Conclusion Resistance of Acinetobacter baumannii is obvious in our hospital,empirical antibiotic treatment should be directed against this pathogen according to the results of drug sensitivity test,and effective measures should be taken to prevent the occurrance and spread of multidrug-resistant and pandrug-resistant strains.
