月经周期对子宫腺肌病子宫动脉血流动力学的影响——经阴道彩色多普勒超声改变 被引量:1

摘要 目的观察子宫腺肌病(AM)患者月经周期中子宫动脉血流动力学变化规律与特点。方法应用经阴道彩色多普勒超声(TV—CDFI)分别于月经周期的卵泡早期、卵泡成熟期、黄体中期、黄体晚期观测子宫腺肌病患者及健康生育期妇女子宫动脉阻力指数(RI)及收缩期/舒张末期流速比值(S/D)。结果AM患者双侧子宫动脉月经周期中RI及S/D差异无统计学意义;卵泡早期和卵泡成熟期RI、S/D略高于黄体中期和黄体晚期,但其差异无统计学意义。健康生育期妇女双侧子宫动脉月经周期中RI及S/D差异无统计学意义;卵泡早期RI最高,黄体晚期RI最低;卵泡早期和卵泡成熟期RI、S/D高于黄体中期和黄体晚期(P〈0.05)。结论AM患者双侧子宫动脉间血流动力学无明显差异,月经周期中血流周期性变化消失,RI及S/D较健康生育期妇女降低。 Objective To investigate the law and characteristics of the variations of uterine artery hemodynamics in menstrual cycle in patients with adenomyosis (AM). Methods With transvaginal color Doppler flow imaging (TV - CDFI), systolic maximal velocity (Vs), end -diastolic velocity (Vd), resistance index (RI) and systolic phase / end -diastolic phase (S/D) ratio of uterine artery were determined in the follicle growth phase, follicle maturation phase, midluteal phase and late luteal phase in menstrual cycle in AM patients as well as healthy procreative women. Results There was no significant difference in RI and S/D in bilateral uterine arteries in menstrual cycle in patients with AM. RI and S/D in early follicular phase and mature follicle phase were slightly higher than those in midluteal phase and late lu- teal phase, but the difference was of no statistical significance. There was no significant difference in RI and S/D in bilateral uterine arteries in menstrual cycle. The peak of RI was in early follicular phase, and the nadir was in late luteal phase. RI and S/D in early follicular phase and mature follicle phase were higher than those in midluteal phase and late luteal phase with statistical significance (P 〈 0.05 ). Conclusion There was no significant difference in hemodynamics between bilateral uterine arteries in patients with adenomyosis, who displayed lack of periodic variations of hemodynamics in menstrual cycle and lower RI and S/D than those in healthy procreative women.
