群组看病模式在社区高血压管理中的效果评价 被引量:8

摘要 目的探索适宜社区应用和推广的高血压有效管理模式。方法群组看病的患者组由一组医务人员(临床医生、公卫医生和社区护士各1人)对其采取集中预约看病随访的方式进行管理;对照组选择同社区的患者,采取常规的防治管理,经过2009年1月—12月的观察,比较对照组和群组看病组患者在高血压知识知晓率、健康行为形成率、患者个人技能提高和对卫生服务满意度的差异。结果经过12个月的管理和随访,群组看病组患者的高血压知识知晓率高于对照组(87%,46%,χ2=9.108,P<0.05),健康行为形成率、个人技能提高及对卫生服务的满意度均高于对照组患者(健康行为形成率干预组与对照组比较,χ2=26.15(v=4),P<0.05;个人技能提高及对卫生服务的满意度干预组与对照组比较,χ2=21.27(v=4),P<0.05),组间血压无显著性差异。结论群组看病模式有助于提高患者对高血压病的认识,提高患者的高血压防治知识知晓率、健康行为形成率及个人技能提高和对卫生服务的满意度。 OBJECTIVE To explore efficient management mode of Hypertension Disease in appropriate community. METHOD Study group with Hypertension disease patients was managed and follow-up by a team of medical personnel including one clinician,one public health doctor and one community nurse via group medical interview; control group whose patients were selected from the same community was managed in conventional preventive and remedial way,after follow-up from January of 2009 to December of 2009,to explore the differences on patients Hypertension Disease knowledge awareness rate,patients new healthy behavior rate,personal skills improvement and patients’ satisfaction on health service rate between two groups. RESULT After management and follow-up from January of 2009 to December of 2009,patients Hypertension Disease knowledge awareness rate(87%)in study group was higher than rate(46%)in control group,with χ^2=9.108,P value 0.05; patients new healthy behavior rate,personal skills improvement and patients’ satisfaction on health service rate in study group were both higher than rates in control group(all P values are below 〈0.05).CONCLUSION Group medical interview is an efficient management mode which increased patients Hypertension Disease knowledge awareness rate,patients new healthy behavior rate,personal skills improvement and patients’ satisfaction on health service rate.
