摘要
目的探讨应变率成像用于评价慢性肾脏病(CKD)患者左室收缩及舒张功能的价值。资料与方法选取71例CKD患者(CKD3期23例,CKD4期22例,CKD5期26例),同时选取20例正常人作为对照组,利用常规超声心动图及应变率成像观察心脏结构及基底部各节段收缩、舒张早期及舒张晚期应变率指标。结果 CKD患者左房内径、左室舒张末期内径、室间隔厚度、左室后壁厚度、左室心肌质量指数和室壁相对厚度大于对照组,CKD4期及CKD5期左室射血分数低于对照组(P<0.05)。CKD3期舒张早期应变率与舒张晚期应变率比值(SRe/SRa)低于对照组(P<0.05)。CKD3期各节段收缩期应变率(SRs)、SRe、SRa与对照组比较,差异无统计学意义(P>0.05);CKD4期各节段SRs及SRe低于对照组;CKD5期各节段SRs、SRe及SRa低于对照组及CKD3期组,除后间隔及前间隔外其他节段SRs低于CKD4期组,各节段SRe均低于CKD4期组(P<0.05)。结论应变率声像能够有效地评价慢性肾脏病患者左室收缩及舒张功能。
Purpose To assess the value of strain rate imaging in evaluating left ventricular systolic and diastolic function in chronic kidney disease(CKD).Materials and Methods A total of 71 patients with CKD(23 patients were in stage 3 of CKD,22 patients were in stage 4 of CKD and 26 patients were in stage 5 of CKD) and twenty healthy controls underwent echocardiography and strain rate imaging for systolic strain rate(SRs),early diastolic peak strain rate(SRe) and late diastolic peak strain rate(SRa) of basal.Results Patients with CKD showed a larger LA,LVEDd,ISVd,LVPWd,LVMI and RWT than control group(P 〈 0.05).Left ventricular ejection fraction of the patients in stage 4 and 5 CKD was lower than control group(P 〈 0.05).The patients in Stage 3 of CKD showed lower SRe/SRa than control group(P 〈 0.05).There were no signif icantly difference in SRs,SRe and SRa between the patients in stage 3 of CKD and control group(P〉0.05).Compared with control group,the SRs and SRe of the patients in stage 4 of CKD decreased significantly(P〈0.05).The SRs,SRe and SRa were reduced in stage 5 of CKD when compared with control group and the patients in stage 3 of CKD,SRa was lower than that in stage 4 of CKD(P 〈0.05).SRs was reduced in stage 5 of CKD than stage 4 except anteroseptal basal and posteroseptal basal(P〈0.05).Conclusion The strain rate imaging can evaluate left ventricular systolic and diastolic function in chronic kidney disease.
