改良鼻中隔缝合联合等离子下鼻甲消融在鼻中隔偏曲矫正术中的选择性应用 被引量:23

摘要 目的鼻中隔偏曲矫正术后使用改良鼻中隔缝合技术,等离子消融下鼻甲,术后不填塞鼻腔,比较其与传统方法的疗效。方法 80例行鼻中隔偏曲矫正术患者分为填塞组和缝合组,行鼻中隔矫正术后,填塞组吸切钻切削下鼻甲后行鼻腔填塞,缝合组行改良鼻中隔缝合、等离子消融下鼻甲,无填塞。比较两组患者术后症状、体征评分。结果填塞组在手术后鼻部疼痛、头痛、吞咽困难、睡眠困难、出血方面的视觉模拟评分法评分大于缝合组(P均<0.05),手术后抽取填塞物时的疼痛感及出血量远大于缝合组(P均<0.05),填塞组术后出现1例再次出血、1例中隔血肿,而缝合组则无。结论鼻中隔偏曲矫正术时行改良鼻中隔缝合,等离子消融下鼻甲,患者术后痛苦小、出血少、避免了鼻中隔血肿等并发症的出现。 OBJECTIVE The aim of this study was to compare the effects of nasal septum suture and inferior turbinate coblation after septoplasty with nasal packing.METHODS Eighty patients who had undergone septoplasty were included in this study.The patients were allocated into tow groups: Packing group,patients in which were packed with mercel in nose after septoplasty and powered inferior turbinatectomy;Suturing group,in which septal suture and inferior turbinate coblation were performed after septoplasty without nasal packing.Postoperative signs and symptoms(Visual analogue scale,VAS) were compared between the two groups.RESULTS The patients of packing group experienced significantly more postoperative nasal pain,headache,dyaphagia,sleep disturbance and bleeding after surgery(P0.05).More pain and bleeding were experienced when compared the pack removal(packing group) with the clearance of nasal cavity(suturing group).One patient with postoperative bleeding and one with septal hematoma were found in the packing group.CONCLUSION Nasal septum suture combined with Inferior turbinate coblation might be a significantly more comfortable,reliable method alternative to nasal packing.
