面颈部双叶皮瓣在颊部软组织缺损中的应用 被引量:3

摘要 目的:探讨应用面颈部双叶皮瓣即刻修复颊部皮肤软组织缺损的可行性及美容效果。方法:对9例面颊部中等面积皮肤软组织缺损(基底细胞癌6例、鳞状细胞癌2例、血管瘤并溃疡1例)患者采用面颈部双叶皮瓣修复。手术边缘:血管瘤为0.2cm,基底细胞癌0.5~0.7cm鳞状细胞癌1cm,切除深度达浅筋膜层。缺损面积最大5.0cm×4.5cm,最小4.0cm×3cm。结果:创面Ⅰ期愈合,无皮瓣坏死,经6个月~5年追踪复查,肿瘤无复发及转移。修复组织质地、颜色与周围皮肤匹配,面部外形较好,美容效果满意。结论:双叶皮瓣行颊部皮肤软组织缺损即刻修复是可行的,可获得较理想的美容效果。 Objective To investigate the feasibleness and the comestic outcome of cheek defects reconstruction using cervicofacial bilobed skin flap at one stage.Methods Nine cases of medial cheeks skin defects were the indication(basal cell carcinomas in 6 patients,Squamous cell carcinomas in 2 patients,ulcerated haemangioma in 1 patient.).The distances from incisions to margin of lesions were as follow: hemangioma was 0.2 cm,basal cell carcinoma was 0.5~0.7cm,Squamous cell carcinoma 1.0cm.The depths of incisions reached superficial fascia layer.The defect sizes ranged from 4.0cm×3.0cm to 5.0cm×4.5cm.Following tumor removal,the flaps were transferred to repair the defects.Results Primary healing was achieved in all cases without complications.The patients were followed up for 6~60 months.No recurrence and metastasis of the tumors occurred.The color and the texture of the flap were the similar with the adjacent skin,and the face contour and cosmetic results were satisfactory.Few complications dornor postoperative deformities.Conclusion The cervicofacial bilobed skin flap is simple and effective for medial cheeks defects reconstruction and provides good cosmesis in selected cases.
