摘要
目的应用单心动周期实时三维超声心动图(sRT-3DE)技术分析房间隔缺损(ASD)的分流量及其影响因素。方法选取30例不合并轻度以上瓣膜反流的单纯继发孔型ASD患者为研究对象,应用sRT-3DE技术获得左、右心室的实时三维图像,采用Va15a分析软件自动测量出右心室及左心室每搏量即肺循环血流量(QpRT-3DE)、体循环血流量(QsRT-3DE),采用TEE测量ASD面积(S),分析Qp/QsRT-3DE、Qp-QsRT-3DE与二维频谱多普勒法所测肺、体循环血流量之比(Qp/Qs2D)、之差(Qp-Qs2D)及S的相关性。结果①Qp/QsRT-3DE与Qp/Qs2D呈正相关(r=0.832,P<0.05),Qp-QsRT-3DE与Qp-Qs2D呈正相关(r=0.668,P<0.05)。②Qp/QsRT-3DE、Qp-QsRT-3DE、Qp/Qs2D、Qp-Qs2D与S呈正相关(r=0.873、0.861、0.801、0.822,P均<0.01)。结论 sRT-3DE能够快速、简单、准确地估测ASD的分流量。
Objective To assess shunt volume in patients with atrial septal defect (ASD) by single cardiac cycle real- time three-dimensional echocardiography (sRT-3DE). Methods Thirty simple secundum ASD patients without middle or severe valvular regurgitation were enrolled. The sRT-3DE was used to obtain images of left and right ventricle. I.eft and right ventricular stroke volume (systemic, pulmonary blood flow QSRT-3DE, QPRT-3DE) were automatically measured with Va15a analytic software. The correlation among Qp/QSRT 3DE, QP--QSRT- 3DE and the area of the defect, the ratio of pulmonary flow to systemic blood flow (QP/QS2D) and their difference (Qp-QS2D) measured by Doppler echocardiography were analyzed. Results ①QP/QSRT-3DE was positively correlated with QP/QS2D(r=0. 832, P〈0.05), QP-QSRT-3DE was positively corre lated with QP-QS2D(r= 0. 668, P〈0.05). ②QP/QSRT-3DE, QP-QSRT-3DE, QP/QS2D, QP-QS2D were positively correlated with the area of the defect (r= 0. 873, 0. 861, 0. 801, 0. 822, all P〈0.01). Conclusion sRT-3DE can easily and accurately estimate the shunt volume in patients with ASD.
