摘要
目的探讨脑室-腹腔分流术治疗脑积水的分流管的选择及提高分流手术疗效的途径。方法回顾性分析2007年9月至2010年3月脑室-腹腔分流术治疗52例脑积水患者的临床资料。52例患者采用高压管分流4例,采用中压管分流25例,采用低压管分流19例,采用可调压管分流4例;其中更换分流管4例。结果术后一周临床症状缓解33例(63.5%,33/52),出院时症状缓解41例(78.8%,41/52)。43例随访3~30个月,脑积水症状改善23例,无显著变化3例,7例症状较术前加重。结论脑室-腹腔分流术仍是治疗脑积水的首选方式;对个体化病人应选择最适合分流压力的分流管进行分流以达到最佳手术效果,如何选择颅内压相适宜的分流装置目前仍需进一步探讨。
Objective To explore the choice of ventriculoperitoneal(V-P) shunt valve in order to enhance curative effect of V-Ps hunt on hydrocephalus.Methods The clinical data of 52 patients with hydrocephalus,who underwent V-P shunt were analyzedr etrospectively,including the curative effect,complication and methods to treat them.Of 52 patients,4 underwent V-P shunt with highp ressure valves,25 with middle pressure valves,19 with low pressure valves and 4 with adjustable valves.Results The clinicals ymptoms were improved in 33 patients(63.5%,33/52) within 1 week after the shunt.The clinical symptoms was improved in 41 patients(78.8%,41/52) on the discharge from hospital.Of 43 patients fallowed up from 3~30 months,33 were improved in hydrocephluss ymptoms,3 unchanged and 7 deteriorated.Conclusions V-P shunt is still first selected method to treat hydrocephalus.The ideal shuntp ressure should be individually selected in order to achieve optimal therapeutic effect when the patients with hydrocephalus are treatedb y V-P shunt.That what is the ideal shunt device suitable for the intracranial pressure in the patients with hydrocephalus should bef urther studied.
