经阴道三维彩色血管能量成像在卵巢癌临床分期中的应用 被引量:1

摘要 目的 探讨经阴道三维彩色血管能量成像(TV3D-CPA)对卵巢癌临床分期的诊断价值.方法 术前采用经阴道三维彩色能量多普勒检测37例卵巢癌患者,评定血管分型,测定肿瘤血管定量参数,即血管指数Ⅵ、血流指数FI及血管化血流指数VFI,结合盆腹腔二维图像,对其进行临床分期,术后根据FIGO2000年标准确定临床分期.结果 I,Ⅱ期和Ⅲ,Ⅳ期卵巢癌血管分型比较差异均有显著性(P<0.05),临床Ⅲ,Ⅳ期VI,FI及VFI值明显高于I,Ⅱ期(P<0.01),卵巢癌的VI,FI及VFI值与FIGO期别分别呈正相关关系.结论 3D-CPA能客观显示各期卵巢癌的血管分布及数量,有助于卵巢癌的分期诊断、治疗和评估手术预后,对妇科手术方案的制定具有指导意义. Objective To evaluate the diagm)stic value of the transvaginal three-dinlensional color power all- giography(TV3D-CPA) in the stage of ovarian cancer. Methods Thirty-seven patients with ovarian cancer were examined with TV3D-CPA preoperatively, to evaluate tile vascular type and determine the quantitative parameters of tumor blood vessels ( vascularity index, VI ; flow index, FI ; vascularization flow index, VFI ). The ovarian cancer were staged according to FIGO2000,combining with pelvic and abdominal cavity two-dimensional image. Results The vascular types of ovarian cancer among stage I , II and III, IV were significant difference( P 〈 0.05 ). VI, FI and VFI in c.linical stage m, IV were significantly higher than that of stage I , II (P 〈0.01 ). Ovarian cancer VI,FI and VFI values had a positive eorrelation with FIGO. Conclusion TV3D-CPA can show the amount and distribution of blood vessel in different stages of ovarian cancer which provides a reliable evidence for stage,treatment and assessment, and also helps for the program of gynecological surgery.
