摘要
目的研究低位直肠癌切除的患者中,保护性造口是否对吻合口漏有预防作用。方法 153例接受低位直肠癌保肛手术患者中,32例行末端回肠造口(造口组),121例未行造口(未造口组);对两组患者的吻合口漏发生率进行统计学分析。结果 153例患者中共有8例发生临床症状的吻合口漏,发生率5.2%。包括:造口组1例(3.1%)、未造口组7例(5.8%),两组差异无统计学意义。未造口组3例行造口手术治疗,未造口组未行再次手术。结论保护性造口不能降低吻合口漏的发生,但能减轻吻合口漏发生后的症状,并减少再次手术的比例。
Objective To investigate whether a defunctioning stoma could reduce anastomotic leakage in patients who received sphincter-preserving operation for low rectal cancer.Methods From September 2009 to January 2011,the clinical data of 153 patients who had received sphincter-preserving operation for low rectal cancer were analyzed.Among them,32 patients received dysfunction stoma(stoma group) and 121 patients received no stoma(non-stoma group).Results Eight patients suffered from symptomatic anastomotic leakage 5.2%.1 in the stoma group(3.1%) and 7 in the non-stoma group(5.8%).There was no statistical difference between the two groups.Three patients underwent reoperation in the non-stoma group,whereas no patient was re-operated on in the stoma group.Conclusions A dysfunction stoma could not reduce the incidence of symptomatic anastomotic leakage,but improves clinical manifestations after leakage and reduces the rate of reoperation.
