联合检测妊娠相关蛋白A在化学发光法检测血清人绒毛膜促性腺激素中的应用 被引量:1

摘要 目的探讨在化学发光法检测血清人绒毛膜促性腺激素(human chorionic gonadotrophin,HCG)中,联合检测妊娠相关蛋白A(pregnancy associated plasma protein-A PAPP-A),以避免假阳性及假阴性的价值。方法收集胶体金早早孕试验为弱阳性的89例患者血清,用化学发光法测定未稀释及1:500倍稀释的HCG水平;用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测PAPP-A。结果化学发光法未稀释及稀释500倍后的结果,在检测值高于试剂检测限时差异有统计学意义(P<0.05);有1例因后带效应产生了假阴性,PAPP-A检测结果验证阳性;4例HCG结果为阳性的标本,PAPP-A检测结果为阴性,加入中和试剂后,HCG结果变为阴性,为嗜异性抗体的干扰。结论对于胶体金早早孕试纸初筛为弱阳性的标本,可采用PAPP-A与HCG联合检测的方法,避免漏诊及误诊。 Objective To explore the value of detecting pregnancy associated plasma protein - A ( PAPP - A ) in chemiluminescence detection of human chorionic gonadotrophin (HCG). Methods Eighty - nine weak positive serum cases with early pregnancy test paper were collected to detect the value of HCG for undiluted and 1 : 500 diluted serum particularly by chemiluminescence, and the value of PAPP -A by enzyme -linked immunosorbent assay (ELISA). Results The results of HCG for undiluted and 1:500 diluted serum were apparently different when the values were outside of the test limit (P 〈0.05) ; One false negative case occurred because of the post banding effect, which was verified by the result of PAPP - A; For the 4 HCG positive cases, the results of PAPP - A were negative. After putting neutralization reagent in the serum, the results of HCG appeared to be negative. Conclusion For the weak positive cases with early pregnancy test paper, combined detection of PAPP - A and HCG can avoid missed diagnosis and misdiagnosis.
