摘要
目的观察比较对剖宫产患者腰-硬联合麻醉不同穿刺间隙的麻醉效果,总结其临床意义。方法选择2009年6月~2011年6月300例剖宫产的产妇,随机分为观察组和对照组,对照组采取L3~4间隙穿刺的腰-硬联合麻醉,观察组采取L2~3间隙穿刺的腰-硬联合麻醉,观察对比两组麻醉效果、PECA(经硬膜外术后镇痛)效果、麻醉过程中的生命体征变化及不良反应。结果 15min的麻脊平面两组对比存在明显差异(P<0.05),具有统计学意义。两组麻醉效果对比存在一定差异(P<0.05),具有统计学意义。两组在麻醉过程中的血压变化及不良反应对比存在一定差异(P<0.05),具有统计学意义。结论采取L2~3间隙穿刺的腰-硬联合麻醉行剖宫产手术对比采取L3~4间隙穿刺的麻醉效果较为满意,且能够提供良好的术后镇痛,但L3~4间隙穿刺的低血压的不良反应比L2~3间隙穿刺的要小,安全性相对高,因此,需要根据临床实际情况及患者的病情进行合理评估选择。
Objective Compare the waist - and epidural anesthesia for cesarean section puncture clearance in patients with different anesthetic effect, summarize its clinical significance. Methods Our hospital from June 2009 to June 2011 300 cases of cesarean section mothers were randomly divided into two groups and the control group and the control group to take the L3-4 lumbar puncture gap - epidural anesthesia, the observation group to L2-3 lumbar puncture gap - and epidural anesthesia, observed and compared two groups of anesthesia,PECA, vital signs during anesthesia changes and adverse reactions. Results 15min flat ridge of Ma significantly different comparing the two groups (P〈0.05), with statistical significance. Compared two groups of anesthesia there are some differences (P〈0.05), with statistical significance. The two groups in changes in blood pressure during anesthesia and adverse reactions were significantly different compared (P〈0.05), with statistical significance. Conclusion L2-3 space to the lumbar puncture - and epidural anesthesia for cesarean section in patients compared to the anesthetic effect of L3,4 lumbar puncture gap - the analgesic effect of epidural anesthesia satisfaction, and can provide good anesthesia, but L3-4 space puncture of hypotension adverse reactions than the L2-3 space puncture is smaller, relatively high security.therefore, the actual situation based on clinical need and make a reasonable assessment of the patient's condition choice.
