摘要
目的探讨经阴道彩色多普勒超声检查在诊断妇科盆腔肿瘤闭孔淋巴结转移中的价值。资料与方法选择经组织病理活检确诊并行广泛性子宫切除+盆腔淋巴清扫术的62例妇科盆腔肿瘤患者,术前采用经阴道彩色多普勒超声检查闭孔淋巴结情况,并与手术病理结果进行对照,评价经阴道彩色多普勒超声诊断盆腔肿瘤的准确性及闭孔淋巴结转移的敏感性和特异性。结果 62例盆腔肿瘤患者中,59例超声检查结果与手术病理结果相符,诊断准确性为95.16%。19例患者病理诊断存在闭孔淋巴结转移,超声诊断16例,假阴性3例;病理诊断无转移43例,超声诊断无转移46例,假阳性3例,两者诊断闭孔淋巴结转移差异无统计学意义(χ2=0.358,P>0.05)。超声对闭孔淋巴结转移的诊断敏感性为84.21%,特异性为93.5%。结论经阴道彩色多普勒超声诊断妇科盆腔肿瘤闭孔淋巴结转移敏感性高、特异性好,具有重要的临床应用价值。
Purpose To investigate the value of transvaginal color Doppler ultrasonography in the diagnosis of obturator lymph nodes metastases of gynecological pelvic tumors. Materials and Methods Sixty-two patients with gynecological pelvic tumor undergoing radical hysterectomy and pelvic lymphadenectomy were included in the study. All the patients underwent transvaginal ultrasonography before surgery to detect obturator lymph nodes, and the findings were compared with surgery. The sensitivity and specificity of transvaginal ultrasonography were assessed. Results Of all patients, 59 of 62 had ultrasound results consistent with surgical findings with the diagnostic accuracy of 95.16%. Nineteen patients with obturator lymph nodes metastases were confirmed by pathology, while only 16 were diagnosed by transvaginal ultrasonography, and false negative in 3. Forty-three patients were confirmed without metastases by pathology, while 46 were diagnosed by ultrasound, and false positive in 3. The sensitivity and specificity of transvaginal ultrasonography were 84.21% and 93.5% respectively. Conclusions Transvaginal ultrasonography has clinical value in the diagnosis of obturator canal lymph nodes metastases of gynecological pelvic tumors.
