摘要
目的了解血吸虫病与乙型肝炎的关系及其流行规律。方法在血吸虫病流行区、控制区和非流行区的城市和农村人群采用Dot-ELISA检测血吸虫CAb、CAg及ELISA法检测HBV5项指标。结果3个试区受检者HBV阳性率分别为20.2%、29.1%、32.1%,控制区和非流行区显著高于流行区。3个试区城市比较,非流行区(32.5%)高于流行区(11.7%)和控制区(16.7%);3个试区农村比较,以控制区最高。流行区和控制区农村高于城市,而非流行区农村和城市相同。在流行区农村,血吸虫感染阳性人群HBV阳性率(23.4%)与未感染血吸虫人群阳性率(24.3%)无显著差异,但发现合并感染都集中在CAb形成的人群中。结论各地HBV感染与环境因素及该地区对疾病的认识与预防有关。血吸虫病与乙型肝炎似乎无明显的内在联系,但有外在的协同作用。
Objectives To investigate the relationship between schistosomiasis japonica and HBV infections, and to search the pattern of the epidemic. Methods An endemic area, a controlled area and a non-endemic area of schistosomiasis were chosen as the pilot areas. The people from city and countryside in each pilot area were detected comparatively using Dot-ELISA for schistosome an tibody (CAb) and antigen (CAg), and ELISA for HBVM 5 indexes. Results The HBVM positive rates of three pilot areas were 20. 2%, 29.1 % and 32. 1% respectively. The HBVM positive rates in controlled and non-endemic areas were higher than that in endemic area. The HBVM positive rates of city people from three pilot areas were compared and that in non-endemic area (32. 5% ) was higher than those in endemic (11. 7% ) and controlled areas (16. 7 % ). The countryside people from three pilot areas were compared and the HBVM positive rate was the highest in controlled area. The HBVM positive rates in rural area people were higher than those in city in endemic and controlled areas, but that was the same in rural area and city in non-endemic area. The HBVM positive rates in the people of schistosome positive and negative were 23. 4%, 24. 3 % respectively and there were no significant difference between them. However, all combined infections concentranted on the peo ple who were CAb positive. Conclusions Environmental factors and preventive knowledge may give certain impact on the infection of HBV. The internal relationship between schistosomiasis and HBV infection seems to be not clear, but combined infections resu1t in cooperate effect by external factors.
