摘要
目的探讨糖尿病(DM)合并亚临床甲状腺功能减退(SCH)患者肾功能的改变。方法比较93例DM+SCH和154例DM+甲状腺功能水平正常(EUT)患者空腹血糖(FBG)、糖化血红蛋白(HbA1c)、估算的肾小球滤过率(Estimated Glomerular Filtration Rate,eGFR)、尿白蛋白/肌酐比率、及尿白蛋白排泄率(UAER)水平。结果①DM+SCH组尿白蛋白/肌酐比率及尿白蛋白排泄率显著高于DM+EUT组(P<0.05),DM+SCH组肾小球滤过率显著低于DM+EUT组(P<0.05);②尿白蛋白/肌酐比率、及尿白蛋白排泄率与促甲状腺激素(TSH)水平呈正线性相关(r=0.27,r=0.23,P<0.01),eGFR与TSH水平呈负线性相关(r=-0.3,P<0.01)。结论 DM合并SCH患者肾脏损害较甲状腺功能正常患者明显,SCH可能是促进糖尿病患者发生糖尿病肾病的风险之一。
【Objective】 To investigate the changes of renal function in diabetes mellitus(DM) with subclinical hypothyroidism(SCH).【Methods】 Serum Fasting PG(FBG),glycated haemoglobin(HbA1C),Estimated Glomerular Filtration Rate(eGFR),urinary albumin-to-creatinine ratio and Urinary albumin excretion rate were compared between 93 patients with DM combined with SCH and 154 patients with DM combined with euthyroidism(EUT).【Results】(1) The urinary albumin-to-creatinine ratio and Urinary albumin excretion rate levels were significantly higher in DM+SCH group than those in DM+ EUT group(P 0.05).(2) The eGFR level was significantly lower in DM+ SCH group than that in DM+ EUT group(P 0.05).The urinary albumin-to-creatinine ratio and Urinary albumin excretion rate levels were linearly and positively correlated to TSH level,respectively(r =0.27,r =0.23,P 0.01).The eGFR level was linearly and negatively correlated to TSH level(r =-0.3,P 0.01).【Conclusion】 The kidney damage is more obvious in Diabetes with subclinical hypothyroidism patients than that in Diabetes with euthyroidism patients,SCH may be one of the risk factors which promote the development of diabetic nephropathy.
