自发性低颅压综合征的影像学特点

摘要 目的探讨自发性低颅压综合征(SIH)的影像学特点。方法回顾性分析9例SIH患者的临床及影像学资料。结果本组男5例,女4例;均为缓慢起病。7例有体位性头痛,2例为颈肩部疼痛。头颅MRI平扫发现脑干移位4例,其中1例合并小脑扁桃体下疝;硬膜下积液2例,垂体增大2例,窦汇后缘呈弧形改变6例;增强发现7例大脑半球硬脑膜、大脑镰及小脑幕弥漫性均匀强化,"铠甲"样包绕脑组织;1例同时合并上矢状窦血栓形成。脊髓MRI平扫发现颈椎间盘突出、神经根袖囊肿及腰椎管内神经鞘瘤各1例;增强后发现脊膜均匀显著强化3例。结论 SIH影像学特征为"下沉脑"及脑脊膜均匀强化,MRI检查有助于对其的早期诊断。 Objective To explore the imaging features of spontaneous intracranial hypotension ( SIH ). Methods The clinical and imaging data of 9 SIH patients were analyzed retrospectively. Results There were 5 males and 4 females in this group, and the onset of them was chronicity. Sever cases had orthostatic headache and 2 cases had neck shoulder pain. By MRI scan, the abnormalities were found out such as brain stern displacement in 4 cases and 1 of them had cerebellar tonsillar herniation; subdural fluid collection in 2 cases; pituitary significantly increase in 2 cases; the arch-fike change of sinus confluens edge in 6 cases. By MRI contrast-enhanced scan, cerebral hemisphere dural, cerebral falx and tnetorinm cerehelli were diffuse and homogeneous enhanced in 7 cases. The meningeal was thicking, " armor" -like surrounding cerebral tissue; and 1 of them was combined with superior sagittal sinus thrombosis. Cervical disc, nerve root sleeve cyst and lumbar spinal nerve sheath tumor were found out each for 1 case by spinal MRI scan. Spinal MRI contrast-enhanced scan showed that meninges was significantly enhanced in 3 cases. Conclusion SIH imaging features are the "dumping brain" and homogeneous meninges enhancement. MRI examination will be helpful to the diagnosis in early stage of SIH.
