摘要
目的了解妊娠期糖尿病(GDM)患者血清脂联素水平与胰岛素抵抗的关系,探讨脂联素与GDM发病的相关性。方法采用酶联免疫吸附法测定30例妊娠期糖尿病孕妇(GDM)和30例正常糖耐量孕妇(NGT)空腹血清脂联素水平;同时测定两组空腹胰岛素(FINS),空腹血糖(FPG)及糖化血红蛋白(HbA1 c)水平。用稳态模型胰岛素抵抗指数(HOMA-IR I)来评价胰岛素抵抗程度。结果①GDM组血清脂联素水平明显低于NGT组,差异有统计学意义(P<0.05)。②GDM组FINS、FPG及HbA1 c水平明显高于NGT组,差异有统计学意义(P<0.01)。③GDM组HOMA-IR I明显高于NGT组(P<0.01)。④相关分析结果显示,血清脂联素与孕晚期BM I、FPG、HbA1 c、FINS及HOMA-IR I呈负相关(P<0.01或P<0.05),与孕前BM I无明显相关性。结论低脂联素水平参与GDM患者胰岛素抵抗的形成,GDM与脂联素水平降低密切相关。
Objective To investigate the change of serum adiponectin(APN) and its relationship with insulin resistance in women with gestational diabetes mellitus(GDM).Methods Enzyme-linked immunosorbent assay was used to measure the fasting serum APN levels of 30 cases of women with GDM,30 cases of women with normal pregnancies in late pregnancy.Fasting insulin(FINS),plasma glucose(FPG) and glycosylated hemoglobin A1c(HbA1c) were also measured at the same time.The degree of insulin resistance was measured by homeostasis model assessment(HOMA-IR).Results ①serum APN concentration in patients with GDM decreased significantly as compared with those with normal pregnancies(P〈0.05).②The concentration of FINS,FPG and HbA1c in GDM group were significantly higher than those of normal pregnancy group(P〈0.01).③HOMA-IRI in the GDM group was significantly higher than those in the NGT group(P〈0.01).④Correlation analysis showed that the serum APN levels were negatively correlated with FPG、HbA1c、FINS、HOMA-IRI and BMI in late pregnancy(P〈0.01 or P〈0.05),but no correlation was found between APN and progestational BMI.Conclusion The serum adiponectin level in the patients with GDM decreases significantly.The decreased APN may be involved in the formation of insulin resistance in GDM.
