摘要
目的探讨肋骨爪形接骨板内固定治疗多根多处肋骨骨折的价值。方法对30例多根多处肋骨骨折患者,在全麻气管插管下进行手术,采用肋骨爪形接骨板内固定,肋骨骨折数4~14根,其中17例胸廓塌陷、变形,有明显胸壁软化。4例伴近胸肋关节处肋软骨折用钢丝内固定,根据肋骨骨折的部位设计切口,尽量不切断胸壁肌肉。结果术后胸壁稳定,胸廓塌陷畸形矫正,无呼吸痛,术后3~4月胸部X线复查肋骨骨折线消失,肋骨对位好,双侧胸廓对称,无死亡病例,患者基本恢复正常,疗效满意。结论对多根多处肋骨骨折尤其伴胸壁塌陷者应行切开复位肋骨内固定治疗,爪形肋骨接骨板内固定手术操作简单,创伤小,手术时间短,固定可靠,矫形效果满意。
Objective To evaluate the application of the internal fixation of the claw plate for treating multiple fractures of multiple ribs.Methods 30 patients were treated with the internal fixation of the claw plate from October 2008 to December 2010.The number of broken ribs in each patient ranged from 4 to 14 and there were 17 patients with the collapse and deformity of the thoracic skeleton.Among them,4 cases who suffered from costal cartilage fracture in sternocostal joint were managed by internal fixation of ribs with steel wires.Incisions were designed according to the site of the rib fractures and save the function of muscles of thoracic wall as far as possible.Results The satisfactory fracture reduction was achieved.The thoracic wall was stable and the collapse and deformity of the thoracic skeleton were retrieved after the surgery,there was no pain associated with the breath and no perioperative mortality.The chest radiograph showed that the fracture lines were disappeared three or four monthes later.Conclusion Multiple fractures of multiple ribs should be internally fixed,especially for patients with the collapse of the thoracic skeleton.The procedures of the internal fixation of the claw plate are very simple,and there is less trauma and shorter operating time.The effects were satisfactory without severe complications.
