摘要
目的观察矫正关节突关节"骨错缝"手法治疗神经根型颈椎病的有效性。方法将关节突关节"骨错缝"致椎间孔狭窄的59例神经根型颈椎病患者随机分为手法组(37例)和牵引组(22例),其中手法组采用关节突关节"骨错缝"矫正手法治疗,牵引组采用传统的枕颌布托牵引治疗;两组均治疗3个疗程,观察临床症状、体征和颈椎间孔矢状径变化情况。结果手法组、牵引组临床症状、体征改善率比较,差异有统计学意义(P<0.05)。手法组治疗前后椎间孔矢状径比率差异有统计学意义(P<0.01),牵引组无明显变化(P>0.05);组间治疗后比较,椎间孔矢状径比率差异有统计学意义(P<0.05)。手法矫正关节突关节错缝矫正程度与疗效之间成正相关关系(P<0.01)。结论关节突关节"骨错缝"矫正手法可有效治疗关节突关节"骨错缝"致椎间孔狭窄的神经根型颈椎病。
Objective To observe the effectiveness of correction procedure for zygapophyseal joints in treating cervical spondylotic radiculopathy. Methods Fiftynine patients with cervical spondylotic radiculopathy due to zygapophyseal joints were randomly divided into manipulation group and traction group. Thirty-seven patients in manipulation group were treated by correction procedure, and twenty-two patients in traction group were treated by pillow jaws bhutto traction, with three courses. The symptoms, signs and changes of intervertebral foramina sagittal diameter were observed. Results The improvement rate of symptoms and signs in both groups had significant differences ( P 〈 0.05 ). The intervertebral foramina sagittal diameter in manipulation group had a significant differences between before and after treatment( P 〈 0.01 ), but without significant differences in traction group( P 〈 0.05 ). The rate of intervertebral foramina sagittal diameter was increased in manipulation group after treatment, with a significant difference between the two groups( P 〈 0.05 ). There was positive correlationship between the fissure of displacement and therapeutic effects(P 〈0.01 ). Conclusion The correction procedure has positive effects on cervical spondylotic radieulopathy due to zygapophyseal joints.
