摘要
目的比较脑电双频指数(BIS)指导下七氟烷、芬太尼静吸复合麻醉与丙泊酚、芬太尼静脉复合麻醉在乳房肿块切除手术中的安全性和有效性。方法 80例择期行乳房肿块切除手术患者,按麻醉方法分为七氟烷、芬太尼静吸复合麻醉(A组)和丙泊酚、芬太尼静脉复合麻醉(B组)。记录2组患者麻醉前后、麻醉期间心率(HR)、血压(NBP)、血氧饱和度(SPO2)、呼吸频率(RR)的变化,以及入睡时间、睁眼时间和不良反应。结果 2组入睡时间、睁眼时间比较无统计学差异。A组麻醉期间NBP较麻醉前后明显下降;B组麻醉期间HR、NBP、SPO2、RR较麻醉前后明显下降;麻醉期间B组HR、SPO2、RR较A组下降更明显。2组均未发生术中知晓,在麻醉开始时,静脉注射痛、术后头晕发生率A组明显低于B组。结论 BIS指导下七氟烷、芬太尼静吸复合麻醉与丙泊酚、芬太尼静脉复合麻醉在乳房肿块切除手术中麻醉效果确切,而前者由于对循环和呼吸影响小,不良反应少,更值得推荐临床应用。
Objective To compare the safety and efficacy of balanced anesthesia of Sevoflu- rane and Fentanyl with Propofol and Fentanyl on breast mass resection by bispectral index (BIS). Methods Eighty patients with breast mass resection were divided into Sevoflurane and Fentanyl group (Group A) and Propofol and Fentanyl group (Group B) according to anesthesia method. The HR, NBR, SPO2, RR, falling asleep time, eyes opening time and adverse reactions were recorded before, after and during anesthesia. Results There was no statistic difference in falling asleep time and eyes opening time between the two groups. NBP of Group A during anesthesia obviously decreased than that before and after anesthesia. HR, NBP, SPO2 and RR in Group B during anesthesia obviously decreased than that before and after anesthesia. FIR, SPO2 and RR in Group B during anesthesia obviously decreased than that in Group A. There was no awareness in operation in both two groups. The rates of intravenous injection pain and dizziness after operation at the beginning of anesthesia of Group A were obviously lower than those of Group B. ConClusion The balanced anesthesia of Sevoflurane and Fentanyl, Propofol and Fentanyl on breast mass resection by BIS have exact efficacy. The former has slight influence on circulation and respiration, and few adverse reactions. It's worth to be recommended in clinical application.
