摘要
目的:研究大鼠重症急性胰腺炎(SAP)并发急性心肌损害(AMD)时心肌组织黏附分子(P-selectin、VCAM-1、ICAM-1)基因表达的变化以及丹红注射液对表达的影响.方法:90只S-D大鼠随机分为对照组(A组,n=30)、SAP模型组(B组,n=30)和治疗组(C组,n=30),采用腹腔注射L-Arg的制作SAP模型.检测血清CTnI、CK-MB、P-selectin、VCAM-1和ICAM-1浓度;光镜、电镜下观察心肌组织的病理变化;实时荧光定量PCR检测大鼠SAP成模后4h、8h、12h心肌黏附分子(P-selectin、VCAM-1、ICAM-1)表达产物.结果:实验室检查:B组各时间点血清中CTnI、CK-MB、P-selectin、VCAM-1与ICAM-1较A组明显升高,差异有统计学意义(P<0.05).C组各时间点血清中CTnI、CK-MB、P-selectin、VCAM-1与ICAM-1较B组明显降低,差异有统计学意义(P<0.05).心肌病理评分:B组大鼠心肌组织病理学评分较A组明显升高,差异有统计学意义(P<0.05);C组大鼠心肌病理学评分较B组明显降低,差异有统计学意义(P<0.05).基因表达:B组各时间点大鼠心肌组织黏附分子基因表达较A组明显上调,差异有统计学意义(P<0.05).C组各时间点心肌组织黏附分子(P-selectin、VCAM-1、ICAM-1)基因表达较B组明显降低,差异有统计学意义(均P<0.05).结论:大鼠SAP可并发AMD,AMD的发生与心肌组织黏附分子P-selectin、IVAM-1、ICAM-1的基因表达上调有关.丹红注射液对SAP并发的AMD具有防治作用,其作用机制与下调心肌组织中黏附分子P-selectin、VCAM-1、ICAM-1的基因表达有关.
AIM: To investigate the expression of P-selectin, IVAM-1 and ICAM-1 in acute myocardial damage (AMD) secondary to severe acute pancreatitis (SAP) in rats, and to explore possible mechanisms underlying the protecting effect of Danhong Injection on AMD in rats with SAP. METHODS: Ninety male SD rats were randomly and equally divided into three groups: control group (group A), SAP model group (group B) and treatment group (group C). The rats in groups B and C received two injections of LArg (3.2 g/kg body weight, i.p.) at an interval of 1 h to induce SAP. Artery blood samples were collected at 4, 8 and 12 h after SAP induction. Serum concentrations of CK-MB, CTnI, P-selectin, IVAM-1 and ICAM-1 were measured. The expression of P-selectin, IVAM-1 and ICAM-1 genes in myocardial tissue was detected by qRT -PCR. Myocardial pathological changes were examined by optical and electron microscopy. RESULTS: Compared to group A, serum contents of CK-MB, CTnI, P-selectin, VCAM-1 and ICAM-1 significantly increased in group B (all P0.05). Compared to group B, serum contents of CK-MB, CTnI, P-selectin, VCAM-1 and ICAM-1 significantly decreased in group C (all P0.05). Myocardial tissue morphology was normal in rats of the control group. Myocardial histopathologic score was significantly higher in group B than in group A. In the group C, myocardial pathological changes were alleviated, and myocardial histopathologic score was significantly lower than that in group B. Compared to group A, the expression of P-selectin, VCAM-1 and ICAM-1 significantly increased in group B (all P0.05). Compared to group B, the expression of P-selectin, VCAM-1 and ICAM-1 signi cantly decreased in group C (all P0.05). CONCLUSION: SAP induces AMD possibly by increasing the expression of P-selectin, VCAM-1 and ICAM-1 genes in rats. Danhong Injection is effective in attenuating AMD and its protective mechanism might be related to decreasing the expression of P-selectin, VCAM-1, and ICAM-1 genes.
