摘要
目的评价多层螺旋CT(MSCT)在儿童漏斗胸术前诊断中的价值。方法回顾性分析28例经MSCT检查并诊断为漏斗胸患儿的CT资料,根据CT凹陷指数,将其分为轻、中、重度三组,测量并比较三组患儿胸骨凹陷深度、CT凹陷指数、胸骨凹陷角度、心脏旋转角度及Haller指数五项指标的差异,分析心脏旋转角与其它四项指标的相关性。结果①28例均有不同程度胸骨凹陷,其中轻度凹陷14例,中度凹陷9例,重度凹陷5例;②胸骨凹陷深度、CT凹陷指数、胸骨凹陷角度、心脏旋转角度及Haller指数五项指标在轻、中、重度三组间差异均有统计学意义(均P<0.01);③心脏旋转角度与胸骨凹陷深度、CT凹陷指数及Haller指数呈正相关(rs分别为0.736、0.873、0.881,均P<0.01),与胸骨凹陷角度呈负相关(rs=-0.731,P<0.01)。结论 MSCT可准确显示漏斗胸的畸形程度及心脏受压旋转情况,为临床治疗提供有力的帮助。
Objective To evaluate the preoperative diagnosis of multi-slice CT (MSCT) in children with peetus excavatum. Methods We retrospectively analyzed 28 cases of funnel chest diagnosed by MSCT examination. All the eases were divided into three groups with pathogenetic condition in mild, moderate, serious degree respectively according to the depression of the chest diagnosed by CT. We measured the degree of sternal depression, CT depression index, the angle of sternal depression, cardiac rotation angle, Haller index, compare those five indicators among the three groups, and analyzed the correlation between cardiac rotation angle and the other four indices. Results Varying degrees of pectus excavatum were found in the 28 cases including 14 cases of mild depression ,9 cases of moderrate depression, and 5 cases of serious depression. The five indicators between each of the two groups were significantly different ( P 〈 0.01 ). Cardiac rotation angle was positively correlated to sternal depression depth, CT depression index and Hailer index ( r, =0. 736,0. 873,0. 881 ,P 〈 0.01 ) ,and was negative correlated to sternal depression angle (r, =-0.731 ,P 〈 0.01 ). Conclusion MSCT can accurately display the deformity of funnel chest and the rotation of cardiac compression, to provide effective help for clinical treatment.
