超声引导下坐位右颈内静脉置管的临床应用 被引量:2

摘要 目的探讨超声引导下坐位右颈内静脉置管的可行性及安全性。方法选择确诊的慢性肾衰竭尿毒症期合并急性左心衰竭患者60例,随机分为两组,每组各30例:A组(超声引导组),B组(传统体表定位组)。结果两组患者在性别、年龄等方面无显著差异。经置管后比较,A组置管成功率、一次置管成功率分别为100.00%、96.67%,均显著高于B组70.00%、50.00%(P<0.01)。A组成功置管所需时间及次数均明显少于B组(P<0.01),且A组置管近期并发症明显少于B组(P<0.01)。结论超声引导下坐位右颈内静脉置管是安全可行的,可提高置管成功率,减少患者痛苦及置管近期并发症。 Objective To discuss the feasibility and safety of ultrasound guidance in the right internal jugular vein catheterization for sitting position.Methods 40 patients with chronic uremia complicated with acute left heart failure were randomly divided into A group(ultrasound-guided group) and B group(traditional landmark technique group).Results The total success rate of catheterization and the success rate of one attempt in A group(100.00%,96.67%) were respectively higher than those in B group(70.00%,50.00%),with statistical significance(P0.01).The time and frequency of successful catheterization in A group were less than those in B group(P0.01),and recent complication in A group was less than B group(P0.01).Conclusion The application of ultrasound guidance in the right internal jugular vein catheterization for sitting position is feasible and safe because of elevating the success rate of catheterization and decreasing the recent complication.
