摘要
目的初步探讨应用方体定向软管血肿吸引术治疗老年高血压患者壳核出血的临床效果及意义。方法回顾性分析我院治疗的老年高血压壳核出血患者227例,根据治疗方法分为方体定向吸引术组(微创组)113例及内科保守治疗组(内科组)114例,比较2组治疗效果。结果微创组平均住院时间较内科组明显缩短(14 d vs 26 d),近期功能恢复良好率明显升高(50.0%vs 20.0%),病死率明显下降(6.2%vs 16.7%),并发症(肺内感染、电解质紊乱、急性肾功能不全、再出血)发生率明显下降(P<0.05)。治疗后6个月随访,微创组出血量<50 ml患者治愈率明显高于内科组(57.8%vs 12.8%),差异有统计学意义(P<0.05)。结论方体定向软管血肿吸引术治疗老年高血压壳核出血可明显缩短住院治疗时间,减少并发症发生,降低病死率,改善远期预后。
Objective To evaluate the therapeutic effect of cuhoid stereotactic catheter indwelt aspiration on hypertensive putamen hemorrhage in gerontal patient. Methods The clinical data of 227 gerontal patients with hypertensive putamen hemorrhage were analyzed retrospectively. They were divided into two groups by method of aspiration treatment with cuboid stereotactic catheter indwelt aspiration (minimally invasive group, n : 113 ) and conservative treatment ( medication group,n~ 114). The clinical outcomes of the patients were compared between the two groups. Results Compared with the results of medication group,the average hospital stay of the minimally invasive group was significantly shortened(14 d vs 26 d),the rate of short-term good-funtional recovery was obviously increased(50. 0% vs 20. 0%),the mortality rate significantly declined (6.20/40 w 16. 70/40),the incidence of complications(such as pulmonary infection, electrolyte disturbances,acute renal insufficiency and rehaemorrhage) declined,at the 6th month after treatment the curative rate of patients with hemorrhage volume less than 50 ml was significantly increased (57.8% vs 12.8%),there were statistically significant differences(P〈0.05). Conclusion Com- pared with conservative treatment, the minimally invasive treatment with cuboid stereotactic catheter indwelt hematoma aspiration can significantly shorten the hospital stay, reduce complications, decline the mortality rate, and improve the long-term prognosis.
