摘要
目的:观察中医益气活血法与健脾补肾法预防奥沙利铂相关神经毒性的效果。方法:60例含奥沙利铂方案化疗的消化道肿瘤术后病人,按随机数字表法随机分为单纯化疗组,化疗+益气活血组,化疗+健脾补肾组,每组20例,3组均予含奥沙利铂方案化疗。单纯化疗组不加保护剂;化疗+益气活血组自第1周期化疗开始口服益气活血的中药;化疗+健脾补肾组自第1周期化疗开始口服健脾补肾的中药。均完成8周期化疗,比较两组病人中医症状积分、外周感觉神经毒性、外周神经传导速度情况。结果:化疗中化疗+益气活血组、化疗+健脾补肾组中医症状积分均较单纯化疗组低,均有统计学差异(P<0.01);化疗后化疗+益气活血组外周神经毒性发生率最低,Ⅲ～Ⅳ级神经毒性发生率与单纯化疗组比较有统计学差异(P<0.05);化疗后各组周围神经传导速度减慢,以化疗+益气活血组减慢幅度小,与单纯化疗组及化疗+健脾补肾组比较均有统计学差异(P<0.05)。结论:益气活血法能减轻奥沙利铂相关神经毒性,改善化疗相关症状。
Objective：To observe effect of Yiqihuoxue and Jianpibushen therapy on prevention of oxaliplatin-induced neurotoxicity.Methods：Sixty cases of gastrointestinal tumor patients after operation with oxaliplatin chemotherapy were divided into single group,combined Yiqihuoxue group and combined Jianpibushen group,20 cases in each group.The single group was administered by chemotherapy only while the combined Yiqihuoxue group by chemotherapy and herbs with effect of benefiting vital energy and promoting blood flow,the combined Jianpibushen group by chemotherapy and herbs with effect of invigorating the kidney and splee.After eight cycles chemotherapy,TCM symptoms integral,peripheral sensory nerve toxicity and peripheral nerve conduction velocity were compared among three groups.Results：Compared with the single group,the TCM symptoms integrals of other two groups were lower and statistically significant（P0.01）.Compared with the single group,the incidence rate of 3 and 4 grades neurotoxicity of the combined Yiqihuoxue group was lower and statistically significant（P0.05）.Compared with the combined Yiqihuoxue group,the peripheral nerve conduction velocity of the single group and the combined Jianpibushen group were slower and statistically significant（P0.05）.Conclusion：Therapy method of Yiqihuoxue can reduce oxaliplatin-induced neurotoxicity and improve chemotherapy relevant symptoms.
