重症急性胰腺炎患者二胺氧化酶检测的临床意义 被引量:12

Clinical significance of serum diamine oxidase in severe acute pancreatitis

摘要 目的探讨重症急性胰腺炎(SAP)血清二胺氧化酶(DAO)水平的变化,及其作为监测肠屏障功能指标的临床意义。方法将46例SAP患者按急性生理学与慢性健康状况评分(APACHEⅡ)分为A(<10分,15例)、B(10~20分,19例)、C(>20分,12例)3组,D组为健康对照组(10例)。采用双抗体夹心ELISA法测定所有入选者血清DAO水平的变化。结果与对照组〔(1.49±0.43)U/ml〕比较,A组〔(2.03±0.56)U/ml,t=2.59,P<0.05〕、B组〔(3.09±1.17)U/ml,t=4.12,P<0.01〕、C组〔(2.17±0.81)U/ml,t=2.38,P<0.05〕SAP患者的血清DAO水平均明显增高,以B组增高最明显。SAP3组间血清DAO水平两两比较,B组高于A组(t=3.22,P<0.01)及C组(t=2.40,P<0.05),C组较A组略高,但差异无统计学意义(t=0.51,P>0.05)。结论血清DAO水平可作为SAP肠黏膜损伤早期诊断的敏感指标,SAP病情加重而DAO出现下降时,提示肠黏膜损伤加重。 Objective To explore the changes of serum diamine oxidase (DAO) in patients with severe acute pancreatitis (SAP) and evaluate its clinical significance used as a potential index for monitoring intestinal mucosal barrier function. Methods According to acute physiology and chronic health evaluation (APACHE) Ⅱ scores, the 46 SAP patients were divided into three groups : group A ( n = 15, APACHE Ⅱ score 〈 10), group B ( n = 19, APACHE Ⅱ score 10 - 20) and group C ( n = 12,APACHE Ⅱ score 〉20) ,and group D (n = 10) was served as healthy control. The serum DAO levels of all the participants in the four groups were measured by double antibody sandwich ELISA, and the results were statistically analyzed. Results Compared with group D [ (1.49 ±0.43) U/ml] ,the serum DAO levels in group A [ (2.03 ±0.56) U/ml,t =2.59,P 〈 0.05] ,group B [(3.09±1.17) U/ml,t =4.12,P〈0.01] and group C [(2.17 ±0.81) U/ml,t =2.38,P〈0.05] were significantly higher; the DAO level in B group was significantly higher than those in group A ( t = 3.22, P 〈 0. 01 ) and group C ( t = 2.40 ,P 〈 0. 05 ) ;the DAO level in group C was slightly higher than that in group A, but there was no significant difference between them ( t = 0.51, P 〉 0. 05 ). Conclusions The serum DAO level might be served as a sensitive marker for early diagnosis of intestinal mueosal injury. If the patients' condition worsens while the serum DAO level lowers by contraries, it suggests that intestinal mucosal injury is exacerbated.
