摘要 目的探讨微创经皮钢板接骨术(minimally invasive percutaneous plate osteosynthesis,MIPPO)结合锁定加压钢板(locking compression plate,LCP)内固定治疗四肢骨折的应用体会。方法总结从2007年9月-2010年12月应用微创经皮锁定加压钢板内固定治疗四肢骨折60例,手术均采用MIPPO技术,运用间接复位技术和经皮插入锁定钢板,最大限度保留骨折断端的血运,避免出现骨不连等各种并发症。结果 60例获得随访,平均随访9个月,平均骨折愈合时间为6.3个月,所有病例切口均顺利愈合,均无感染、骨不连、钢板松动等并发症。按照Johner-Wruhs功能评价方法,分为优、良、中、差,本组病例60例,优53例,良6例,中1例,差0例,优良率为98.3%。结论 MIPPO技术结合LCP内固定治疗四肢骨折具有创伤小、并发症少、骨愈合率高、临床效果满意等优点,是治疗四肢骨折的一种良好方法。 Objective To investigate the clinical application and effect of minimally invasive percutaneous plate osteosynthesis(MIPPO) combined with locking compression plate(LCP) internal fixation in the treatment of extremity fractures.Methods The clinical results of 60 patients with sustained limb fractures undergoing MIPPO and LCP from Sept.2007 to Dec.2010 were sumrrized retrospectively.In order to preserve blood supply of fracture segments and minimize the complications,MIPPO technique with indirect reduction and percutaneous plate insertion was applied in all cases.Results All 60 patients were followed up for 9 months.All fractures had successful bone union with mean time of 6.3 months.There was no complication including wound problem,infection,non-union and failure of internal fixations.According to Johner-Wruhs score,53 cases accessed as being excellent,6 as good,1 as fair,the excellent and good rate was 98%.Conclusion With a less invasion,less complications,higer union rate and better clinical result,MIPPO combined with LCP is a good option for the treatment of extremity fractures.
