摘要
目的 探讨母婴ABO/Rh 血型不合产前诊断的有效策略。方法 对ABO/Rh 血型不合孕妇,在孕期进行免疫抗体动态监测,并采用基因定型技术产前鉴定Rh 基因型。结果 5 例Rh 阴性具有不良孕产史孕妇在本方案中及时得到有效处理,并对她们用基因定型技术进行分析;ABO系统新生儿溶血病占高胆红素血症患儿的12-6 % ,Rh 系统HDN 占其中的4-9‰;本文还得到2219 例孕妇产前血清IgG抗A(B) 效价分布比率概况。结论 制定产前监测和控制HDN 方案,及时检测引起HDN 的免疫抗体,采取适当措施可减少对新生儿的危害,IgG 型抗体效价的高低与新生儿溶血病发生率成正比关系,基因定型分析在产前HDN 诊断中将有巨大潜力。
To study the effective strategy of antenatal diagnosis about maternal-fetal ABO/Rh incompatibility hemolysis to improve fetal outcome.Methods:Besides dynamic monitoring of immune antibody valence in maternal serum,the gene determination analysis was carried out for anteatal determining Rh genotypes. Results: 5 Rh-Negative pregnant women with poor obstetrical history were treated with this protocol,and obtained adequeate therapy in time.The frequency of the hemolytic disease of the newborn(HDN)due to ABO?incompatibility (ABO?HDN),requiring treatment,is with approximately 12.6% of all hyperbiliubinemias in newborns, and it is 4.9‰ for Rh?HDN,IgG anti?A(B) were detected in 2219 pregnants,the distribution outline was obtained from the data.Conclusions:All antibodies that induced HDN were detected in time so that adequate measures could be taken to reduce the effects in the newborn.The antenatal screening and management program currently in use is considered to be reliable. It is suggested that the magnitude of valence of IgG blood type antibody is directly proportional to incicence of HDN.This study has also highlighted the need for more investigations into gene determination that play a key role in antenatal diagnosis of HDN.
