摘要
目的探讨氟伐他汀联合普罗布考对糖尿病心肌病患者8-羟基脱氧鸟苷酸(8-Hydroxy-guanin,8-OhdG)水平及心功能的改善情况。方法①73例糖尿病心肌病(diabetic cardiomyopathy,DC)患者随机分为治疗组(37例)和对照组(36例),两组都在糖尿病饮食控制基础上给予拜阿司匹林100 mg/d,消心痛10 mg,3/d,缬沙坦80~160 mg/d口服;胰岛素皮下注射,将血糖控制理想水平;治疗组加用氟伐他汀40 mg/d联合普罗布考0.5 g,2次/d,治疗时间均为6个月,16周为1疗程。②两组治疗前和治疗16周后采用酶联免疫吸附剂测定(enzyme-linked immunosorbent assay,ELISA)法测定其血清8-OHdG水平,并与40例健康体检者进行比较。③采用双盲法通过超声心动图评价DC治疗前后心脏结构和功能。④采用常规12导联心电图观察ST-T改善情况。结果①DC组8-OHdG水平明显高于正常对照组(P<0.05);②治疗组治疗前后8-OHdG水平具有统计学差异(P<0.05),对照组治疗前后无统计学差异,治疗组明显优于对照组(P<0.05);③治疗组治疗后心电图ST-T改善明显优于对照组(P<0.01);④DC治疗组患者心脏收缩功能和舒张功能明显改善,自身比较及与对照组比较差异均具有统计学意义(P<0.05)。结论 8-OhdG在DC发病中具有重要作用,氟伐他汀用于治疗DC安全有效。
Objective To investigate the effects of combined Fluvastatin-Probucol treatment on the cardiac function and 8-Hydroxy-guanin(8-OhdG) of patients with diabetic cardiomyopathy(DC). Methods A total of 73 DC patients were randomly divided into the control group(36 cases) and treatment group(37 cases).Both groups were treated with Valsartan 80-160 mg po qd,Aspilin 100 mg po qd,isosorbide dinitrate 10 mg po tid and hypodermic injection of insuline to control the blood glucose at an ideal level.The treatment group was additionally administrated with Fluvastatin 40 mg po qd and Probucol 0.5 g po bid for 6 months,and 16 weeks as a course.Serum 8-OhdG levels were tested by enzyme-linked immunosorbent assay(ELISA) before and after treatment and compared with 40 healthy controls.Double blind and echocardiographic method was used to compare the cardiac structure and function before and after treatment.Routine 12-heads electrocardiogram was utilized to observe the ST-T improvement. Results Serum 8-OHdG levels in DC group was significantly higher than that in normal control(P0.05).There was significant difference of serum 8-OhdG levels in treatment group and no difference in control group before and after treatment.The serum 8-OhdG levels in the treatment group were significantly better than those in control group(P0.05).The ST-T of electrocardiogram was significantly improved in the treatment group and better than that in the control group(P0.01).The systole and diastole function was significantly improved in the treatment group compared with itself and the control group(P0.05). Conclusion Serum 8-OHdG plays an important role in the pathogenesis of DC.Fluvastatin is effective and safe in DC treatment.
