门诊乙肝患者和经拉米夫定治疗后乙肝患者基因分型和HBV P区耐药突变检测分析 被引量:6

摘要 目的探讨门诊乙肝患者和经拉米夫定治疗后乙型肝炎患者HBV P区耐药突变区别。方法采用荧光定量PCR结合DNA测序技术对患者进行HBV分型和HBV P区耐药突变点进行检测分析。结果门诊乙肝患者55例和经拉米夫定治疗后乙肝患者57例在HBV基因型上并没有显著差异,其中门诊乙肝患者主要存在拉米夫定(6例,10.9%)、阿德福韦酯(3例,5.4%)耐药突变株,没有替比夫定和恩替卡韦突变,而经拉米夫定治疗后乙肝患者HBV P区主要存在拉米夫定位点,共24例(42.1%),阿德福韦酯4例(7.0%),其他突变3例(5.3%)。结论未经治疗的门诊乙型肝炎患者仍然需要进行耐药检测,以得到最佳治疗方案,而经过治疗后的乙型肝炎患者存在大量的拉米夫定耐药株,这对于患者的诊断、制定治疗方案和监测预后具有重要意义。
