摘要
目的 探讨计算机断层扫描三维重建技术(3D-CTA)在自发性颅内出血中的应用价值.方法 分析2007年3月至2011年8月72例自发性颅内出血患者CTA检查结果,结合DSA造影结果及手术,评价CTA在自发颅内出血中的应用价值.结果 72例自发性颅内出血患者,动脉瘤27例,CTA发现25例,17例根据CTA提示直接开颅手术,4例经CTA发现动脉瘤后急诊行栓塞治疗;脑AVM19例,首次CTA检查发现14例阳性,3例首次CTA及DSA检查结果均为阴性,1年后CTA复查发现异常血管团,经DSA检查证实后手术治疗.2例首次及2周后CTA检查阴性,但DSA检查阳性.海绵状血管瘤4例,CTA不显影,MRI+增强及手术病理证实.烟雾病5例,静脉畸形2例,为CTA与DSA证实.其余15例CTA、DSA未见明确病因.结论 CTA对自发性颅内出血是一种无创、快捷有效、安全的影像学方法.
Objective To assess the value of three -dimensional computed tomographie augiography (3D -CTA ) in acute nonhypertensive spontaneous intracranial hemorrhage. Methods A retrospective study included 72 patients with acute nonhypertensive spontaneous intraeranial hemorrhage, which all cases were examined with CTA comparing with the results of the digital subtraction angiography (DSA) and the finding of the surgical treatment. Result In 72 patients, 25 of the 27 aneurysms were detected by CTA which 17 cases were performed surgical operation and 4 cases by endovaseular coiling directly under the leading of CTA images. Nineteen patients were diagnosed as AVM, which 14 cases were confirmed in the first CTA examination. Three cases were found negative results in the first CTA and DSA examination while the following up CTA were positive and confirmed by DSA and operations. Two cases were found negative results in the first and second CTA two weeks later, but small AVM's were seen in DSA images. Four cases negative in CTA examination were cavernous angioma and were confirmed by operation. Five cases with moyamoya diseases and 2 venous malformations were diagnosed by both CTA and DSA. Fifteen patients had negative finds both in CTA and DSA, in which survivors had no rebleeding during 2 years following up. Conclusion 3D - CTA is a non - invaded, rapid, safe and effective method to find the cause of acute nonhypertensive spontaneous intracranial hemorrhage.
