摘要
目的:探讨血浆N-末端脑钠肽前体(NT-proBNP)水平与充血性心衰(CHF)患者房颤发生及左室重构的关系。方法:选择我院2010年5月~2011年5月收治的CHF患者60例(研究组),选择同期我院健康体检者60例为对照组,检测两组血NT-proBNP,左室舒张末期内径指数(LVIDdI)、左室射血分数(LVEF)、左室质量指数(LVMI),并进行统计学比较。结果:研究组患者血NT-proBNP、LVIDdI、LVMI明显高于对照组,LVEF低于对照组,两组比较差异有统计学意义(P<0.05或P<0.01);研究组不同级别心功能衰竭患者,血NT-proBNP水平差异有统计学意义(P<0.05);研究组发生房颤患者血NT-proBNP水平明显高于未发生房颤者,差异有统计学意义(P<0.05);多元分析显示血浆NT-proBNP水平与房颤、左室质量指数(LVMI)呈正相关(r=0.279、0.674,均P<0.05),与左室射血分数(LVEF)呈负相关(r=-1.638,P<0.05)。结论:充血性心力衰竭患者血NT-proBNP水平明显升高,并且血NT-proBNP水平与房颤、心衰程度及左室重构有重要关系,可作为临床评价慢性充血性心力衰竭的重要指标。
Objective: To investigate the relationship of plasma N-terminal pro-brain natriuretic peptide(NT-proBNP) levels of congestive heart failure(CHF) patients with atrial fibrillation(AF) and left ventricular remodeling occurs.Methods: 60 patients with CHF in our hospital from May 2010 to May 2011 were selected as study group,60 cases of healthy over the same period were selected as control group.Blood NT-proBNP,left ventricular diastolic diameter index(LVIDdI),left ventricular ejection fraction(LVEF),left ventricular mass index(LVMI) of two groups were detected and compared.Results: The serum NT-proBNP,LVIDdI,LVMI of study group was significantly higher than those of control group and LVEF was lower than that of the control group,the difference was statistically significant(P0.05 or P0.01);in study group,the NT-proBNP of different levels of heart failure patients were significantly different(P0.05),and NT-proBNP level of patients with AF was significantly higher than those without AF,the difference was statistically significant(P0.05);multivariate analysis showed that plasma NT-proBNP levels was positively correlated with AF and LVMI(r=0.279,0.674,P0.05),and negatively correlated with LVEF(r=-1.638,P0.05).Conclusion: NT-proBNP level of patients with CHF is high,and it has an important relationship with AF,heart failure degree and left ventricular remodeling.It can be an important evaluation indicator of CHF.
