摘要
目的探讨老年原发性高血压伴左室肥厚(EH-LVH)和左室扩大(EH-LVD)的相关危险因素。方法采用PCR-LDR技术对HLA-DRB1 rs9269186、rs3135388位点进行基因分型,通过二维超声和M型超声检查有无LVH和LVD。结果 rs9269186、rs3135388基因型及等位基因频率在EH伴有LVH组、LVD组或LVM/h组与对照组间差异均无统计学意义(P>0.05)。腰围(WL)、腹围(AC)在EH-LVD+组高于EH-LVD-组(P<0.05);体重指数(BMI)、WL、AC随LVM/h比值增高呈上升趋势,存在统计学意义(P<0.001),经多元素Logistic回归分析后BMI及AC仍与LVM/h相关联〔OR依次为2.417(95%CI:1.873~4.294),2.936(95%CI:1.113~7.747)〕。结论 BMI、AC可能是老年人EH左室重构的危险因素。HLA-DRB1 rs9269186,rs3135388单核甘酸多态性与老年EH左室重构无关。
Objective To investigate the related risk factors of left ventricular hypertrophy(LVH) and left ventricular dilation(LVD) in elderly essential hypertension(EH).Methods HLA-DRB1 single nucleotide polymorphisms(SNP,rs9269186 and rs3135388) were determined in 148 EH elderly subjects of Chinese Han population.Two SNPs were genotyped by multiplex polymerase chain reaction-ligase detection reaction.LVH and LVD were determined by the measurements of echocardiogram.Results There was no significant difference in genotype or allele frequencies of rs9269186 and rs3135388 between LVD,LVH,LVM/h groups and control groups(P〈0.05).The levels of waistline(WL) and abdomen circumference(AC) in EH-LVD+ group were significantly higher than those in EH-LVH-group(P〈0.05).There was no difference of Clinical information and SNP between EH-LVH+ group and EH-LVH-group.The levels of body mass index(BMI),WL,AC were statistically increased with the increasing of ratio of LVM/h(P〈0.001).Multivariate logistic regression analysis showed the relationship between BMI,AC and LVM/h(OR=2.417(95%CI:1.873~4.294),2.936(95%CI:1.113~7.747)).Conclusions BMI and AC may be risk factors of EH related left ventricular remodeling in elderly patients.HLA-DRB1 rs9269186,rs3135388 may have no relationship with left ventricular remodeling.
