摘要
目的探讨锁定钢板内固定与传统解剖钢板内固定治疗Schatzker Ⅵ型胫骨平台骨折的临床疗效与预后效果,为相关骨折的治疗提供参考。方法将SchatzkerⅥ型胫骨平台骨折患者80例根据入院顺序分为治疗组与对照组,每组各40例。两组内固定前、后处理方法相同,对照组采用传统解剖钢板内固定,治疗组采用锁定钢板内固定。结果治疗组术后与术后3个月胫骨平台内翻角(TPA)角度与对照组相比无明显变化,差异无统计学意义(P>0.05),但后倾角(PA)角度明显增大,与对照组比较,差异有统计学意义(P<0.05)。随访3个月进行观察,治疗组的患肢膝关节活动度与患肢膝关节功能评分(HSS值)均优于对照组,差异有统计学意义(P<0.05)。结论相对于传统解剖钢板,锁定钢板内固定治疗Schatzker Ⅵ型胫骨平台骨折能有效改善后倾角,有利于患者术后患肢膝关节的恢复,值得推广应用。
Objective To investigate the clinical effects and prognosis of locking plate fixation and traditional anatomy plate fixation in the treatment of Schatzker Ⅵ tibial plateau fracture,in order to provide reference for the treatment of related fracture.Methods 80 cases of patients with Schatzker Ⅵ tibial plateau fractures were equally divided into treatment group and control group,each group had 40 cases.Processing methods of the two groups before and after internal fixation were similar.The control group was treated with traditional anatomical fixation,the treatment group was treated with locking plate fixation.Results The angle of TPA was compared between the two groups postoperation and 3 months after operation,there was no significant difference（P 0.05）,but the angle of PA of treatment group was significantly larger than that of control group,the difference was statistically significant（P 0.05）.After 3 months＇ follow-up observation,the affected limb knee-joint ROM and the affected limb knee-joint function grade（HSS values） of the treatment group were significantly better than those of the control group,the differences were statistically significant（P 0.05）.Conclusion Compared with traditional anatomical plate,locking plate fixation in the treatment of Schatzker Ⅵ tibial plateau fractures can improve the angle of PA effectively.It is conducive to the recovery of affected limb knee-joint after operation,which is worthy of clinical application.
