后腹腔镜泌尿外科手术应用不同充盈压对手术安全性的研究 被引量:1

摘要 目的探讨免水囊扩张低CO2充盈压对后腹腔镜泌尿外科手术的安全性。方法选择美国麻醉师协会(ASA)分级I~Ⅱ级行后腹腔镜泌尿外科手术患者100例,按照随机数字表法分为两组:免水囊扩张低CO2充盈压组(A组)、标准充盈压组(B组),每组50例。监测气腹前及气腹后30、60min动脉血气的变化。结果两组患者气腹后30、60min的pH值较气腹前均降低(A组:7.33±0.03、7.29±0.03比7.44±0.03;B组:7.32±0.03、7.24±0.03比7.44±0.03),动脉血二氧化碳分压(PaCO2)较气腹前均升高[A组:(44±4)、(51±4)mmHg(1mmHg=0.133kPa)比(36-1-3)mmHg;B组:(49±5)、(55±5)mmHg比(37±3)mmHg],差异有统计学意义(P〈0.05);B组气腹后30、60min的pH值较A组明显降低,PaCO2较A组明显升高,差异有统计学意义(P〈0.05)。A组和B组气腹后30、60min高碳酸血症发生率分别为12%(6/50)、42%(21/50)和32%(16/50)、76%(38,50),两组比较差异有统计学意义(P〈0.05或〈0.01)。结论免水囊扩张低CO2充盈压对后腹腔镜泌尿外科手术较标准充盈压手术更安全。
