摘要
目的:观察甲苯胺蓝介导的光动力对大鼠口腔牙周致病菌的治疗效果。方法:20只大鼠均在上颌第一磨牙用牙周致病菌(主要为牙龈卟啉单胞菌)接种创建口腔大鼠牙周病模型。6周后,用甲苯胺蓝1.0g/L+光强度48J/cm2+光剂量212mW/cm2对大鼠进行治疗,观察前后灭菌效果,临床症状(菌斑指数和龈沟出血指数),并进行组织学检查观察疗效。结果:光动力治疗后,细菌明显减少,细菌总菌落数和牙龈卟啉单胞菌(pg)菌落数从(947±28.79)个和(591±21.17)个分别减少为(343±26.06)个和(181±25.48)个,P<0.01。光动力治疗后菌斑指数(PLI)从3减少为1,龈沟出血指数(SBI)从4减少为1。组织学检查未有牙槽骨损伤,炎细胞减少。结论:研究发现甲苯胺蓝介导的光动力疗法能够有效治疗牙周病,与传统方法相比,光动力同样能有效杀灭牙周致病菌(主要为pg),减少炎症,且对正常组织没有损伤,是牙周病常规治疗的有效补充,可能用于牙周病的辅助治疗手段。
Objective: To investigate the photosensitization of periodontal bacteria in rats. Methods: Periodontitis was developed by submerging ligatures at the subgingival region of maxillary molars in 20 rats. Six weeks later, the infection sites were treated with 1 mg/ml of toluidine blue plus 48 J/cm^2 at 212 mW/cm^2 red laser irradiation. After infection, we observed clinical signs of gingival inflammation including an increase of redness and bleeding on probing. Before and after treatment microbiological monitoring was performed. The therapeutic efficacy was assessed by evaluating the reduction of total bacterial flora and histological changes of periodontal tissues. Results: Significant reduction of total bacterial flora from (947±28.79)to (343±26.06)and pg flora from (591±21.17) to (181±25. 48) were achieved by photodynamie therapy. The signs of inflammation that accompanied periodontitis were greatly reduced without any obvious injury to host tissues. PLI was from 3 to 1 and SBI was from 4 to 1. Conclusion: Toluidine blue-mediated photodynamic therapy could effectively treat periodontitis in rats and has high potential in clinical application.
