丹参酮注射液对不同中医证型心肌再灌注损伤的保护作用及其机制 被引量:2

摘要 目的:观察丹参酮注射液对心肌再灌注损伤的保护作用及可能的作用机制。方法:选择行冠状动脉介入治疗(PCI)的患者60例,随机分为常规对照组、西药治疗组和丹参酮治疗组(分实证、虚证二型),分别在术前,术后24h,术后7d记录炎症因子(IL-6、IL-10、TNT-α)、氧化应激因子(NO、SOD、MDA)等指标的变化情况。结果:PCI术后24h与术前比较,炎症因子(IL-6、TNT-α升高,IL-10降低)、氧化应激因子(MDA升高,NO、SOD降低)水平均有显著改变(P<0.05)。PCI术后7d,西药治疗和丹参酮治疗组的炎症因子、氧化应激因子水平的改善变化优于常规对照组(P<0.05)。丹参酮治疗组与西药治疗组比较,炎症因子、氧化应激因子的改善变化均无显著差异(P>0.05),丹参酮治疗对实证组的炎症因子和氧化应激因子的改善作用略优于虚证组(但P>0.05)。结论:丹参酮注射液对心肌再灌注损伤有一定的保护作用,调节炎症因子和氧化应激因子水平是其可能的作用机制;丹参酮注射液对实证型的PCI患者可能更有效。 Objective injury and its mechanism. To evaluate the protective effect of tanshinone injection on myocardial reperfusion Methods 60 patients with percutaneous coronary intervention (PCI) were randomly divided into control group, drug treatment group and tanshinone treatment group (divided into the empirical and deficiency syndromes). The changes of inflammatory cytokines (IL-6, IL-10, TNT-α) and oxidative stress factors (NO, SOD, MDA) of all groups were compared before surgery, 24 hours after surgery and 7 days after surgery. Results The level of indexes of 24 hours after PCI such as inflammatory cytokines (IL-6 and TNT-α increased,IL-10 declined) and oxidative stress factors (NO and SOD declined, MDA increased) were changed significantly (P 〈 0.05), compared with the indexes of the pre-operation. 7 days after surgery, the inflammatory cytokines (IL-6, IL-10, TNT-α ) and oxidative stress factors (NO, SOD, MDA) of drug treatment and tanshinone treatment groups were improved better than control group. The level of inflammatory cytokines (IL-6, IL-10, TNT-α ) and oxidative stress factors (NO, SOD, MDA) had no obvious difference between Tanshinone treatment group and drugs treatment group (P 〉0.05). The level of inflammatory cytokines (IL-6, IL-10, TNT-α) and oxidative stress factors (NO, SOD, MDA) of the empirical syndrome group treated with tanshinone, for 7 days after PCI, were better than deficiency group (but P 〉 0.05). Conclusion Tanshinone injection has a protective effect on myocardial reperfusion injury; the mechanism may be related with that it can regulate the level of inflammatory cytokines and oxidative stress factors. Tanshinone injection has a better effect on the patients who will undergo PCI with empirical syndrome of TCM
