高血压脑出血早期血肿扩大59例分析 被引量:7

摘要 目的分析高血压脑出血早期血肿扩大的临床因素、病理机制及对患者预后的影响。方法回顾性分析2005年2月至2010年11月高血压脑出血早期血肿扩大患者59例的临床资料,随机抽取高血压脑出血无血肿扩大59例作为对照组,对脑水肿、脑梗死体积以及预后进行分析对比。结果血肿早期扩大组脑水肿带体积及脑梗死体积明显大于血肿未扩大组,GOSⅠ~Ⅲ级明显差于血肿未扩大组(χ2=16.89,P<0.01),ADL完全恢复率明显低于未扩大组(χ2=9.43,P<0.01),差异有统计学意义。结论高血压脑出血早期扩大会加重患者的脑水肿及脑梗死,增加患者的病残率及病死率,预防脑出血早期血肿扩大是提高患者的生存质量的关键。 Objective To analysise the clinical factors of the Hypertensive intracerebral hemorrhage with early hematoma enlargement and its impact on the prognosis.Methods 59 cases of clinical data of hypertensive cerebral hemorrhage early hematoma enlargement were analyzed retrospectively,other 59 patients who had with c cerebral hemorrhage were choosed as ontrol group(cerebral hemorrhage without hematoma expansion.Results Volume of cerebral edema and infarction volume in early hematoma expansion group was significantly larger than that in control group,GOS I^III had a significant difference between two group(χ2=16.89,P0.01),ADL complete response rate was sinificantly lower than that in control group(χ2=9.43,P0.01).Conclusion The early hematoma enlargement by Hypertensive intracerebral hemorrhage will increase cerebral edema and cerebral infarction in patients with increased patient morbidity and mortality.Prevention of cerebral hemorrhage early hematoma enlargement is the key to improve the quality of life of patients.
