摘要
目的:探讨冠桥固定重症牙周炎松动牙对牙龈微循环和牙周组织影响。方法:选重症牙周炎患者40例,分为2组,对照组用牙周洁治和刮治,固定组在上述基础上作松牙固定,2组患者分别用激光多普勒血流仪测定治疗前、治疗后1周、治疗后3个月GBF,刚果红染色检测龈下菌斑形态学变化,并测定牙龈指数(GI)、菌斑指数(PLI)和牙周袋深度(PD)、附着丧失(AL)。结果:治疗后1周时固定组GBF和GI、PLI、PD、AL及杆菌、螺旋体和梭状菌比率与对照组有显著性差异(P<0.01);在治疗1周时固定组GBF比治疗前明显增加,GI、PLI、PD、AL和杆菌、螺旋体和梭状菌比率明显降低;在治疗3个月时,固定组GBF明显大于1周时,杆菌、螺旋体、梭状菌比率明显低于1周时。结论:冠桥固定重症牙周炎松动牙可使GBF明显增加,改善牙龈微循环,使龈下菌群向非致病型转变,其疗效与调节牙龈微循环和龈下菌群有密切关系。
Objective: To study the effects of gingival microcirculation and periodontal tissue of ceramic restoration splint on tooth mobility of severe periodontitis. Methods: 40 cases with periodontitis were selected and divided into 2 groups, group periodontal splint (treated by scaling and root planning and periodontal splint) and group control (treated by scaling and root planning). GBF value of each group pre and post-- therapy was evaluated by laser doppler flowmeter. The subgingival microorganisms in each periodontal pocket base were stained with 2% Congo red and pure hydrochloric acid and a proper field of vision was selected for measurement. And gingival index(GI), plaque index (PLI), pocket depth (PD), and attachment loss (AL) of different groups between therapeutic beforeand after 1 week and 3 month were also measured. Results: There were significant differences of GBF, GI, PLI, PD and the percentages of coccoid cells (Cocci), straight rods (Rods), fusiforms (Fusi) and spirochetes (Spiro) be- tween group periodontal splint and group control. The GBF value of group periodontal splint were greater after ther- apeutic 1 week than before, but the GI, PLI, PD, AL and the percentages of Cocci, Rods, Fusi and Spiro reduced more severely than before. The GBF value of group periodontal splint after therapeutic 3 months increased compared with therapeutic 1 week, but the percentages of Rods, Fusi and Spiro reduced severely compared with therapeutic 1 week. Conclusion: There were more therapeutic effects of periodontal splint on the GBF and tooth mobility of severe periodontitis, which improved the gingival circulation, and changed the pathogenic subgingival bacteria to nonpatho- genic bacteria. The therapeutic mechanism of periodontal splint to tooth mobility was related to regulate the gingival microcirculation and subgingival bacteria.
