摘要
目的 探讨脑静脉畸形的MRI表现。材料与方法 9例脑静脉畸形均有MRI及MRA检查 ,MRI行常规平扫及轴位增强扫描。MRA采用 2D TOF或 3D MOTSA法。结果 小脑 4例 ,额叶 3例 ,颞叶 1例 ,大脑大静脉瘤伴中线血管畸形 1例。典型者引流静脉显示长T1短T2 低信号流空影 ,髓静脉网呈长T1低信号长T2 高信号。增强扫描呈“海蛇头”样表现 ,即多支细小扩张的髓静脉呈辐射状向 1条或 2条引流静脉汇聚。MRA可显示髓静脉及引流静脉的整体观及引流方向 ,但显示髓静脉数目较增强扫描少。结论 MRI结合MRA可准确诊断脑静脉畸形及其并发症 ,可取代脑血管造影检查。
Objective To study MRI manifestations of cerebral venous malformation (CVM).Materials and Methods MRI and MRA were performed with a 1.5T MR unit in 9 cases with CVM.Results Lesions were located in cerebellum (n=4), frontal lobe (n=3), temporal lobe (n=1) and Galen vein (n=1). Main drain veins demonstrated hypointensity on T 1WI and T 2WI, while medullary veins showed hypointensity on T 1WI and hyperintensity on T 2WI. After enhancement, numerous radiating medullary veins gathered together into one or two larger drain veins and flowed into superficial or deeper veins, demonstrating so called “Caput Medussa” or “Umbrella” appearance. MRA could show the general portrait of CVM and the direction of vein draining, though the number of medullary veins on MRA was less than that seen on enhanced MRI.Conclusion Coupled MRI with MRA, CVM and its complications can be accurately diagnosed, therefore, it is possible to replace cerebral angiography.
