摘要
目的探讨肠外营养(PN)、肠内营养(EN)及肠外-肠内联合营养(PN+EN)支持对高血压脑出血术后病人体液免疫功能的影响。方法筛选高血压脑出血术后病人90例,随机分为PN组、EN组及PN+EN组,每组30例,分别给予相应营养支持,于入院后1、14d采集外周静脉血测IgA、IgG、IgM、清蛋白及前清蛋白水平,比较3组病人体液免疫水平的变化。结果入院1d时3组各指标检测结果比较,差异无显著性(P>0.05)。入院14d和入院1d比较,PN组各指标检测结果差异无显著性(P>0.05);EN组外周血IgA、IgG和清蛋白含量升高,PN+EN组各指标水平均升高,差异有显著性(t=2.16～3.01,P<0.05)。入院14d时,EN组IgM、清蛋白和前清蛋白含量高于PN组;PN+EN组各指标水平均高于PN组,IgA、清蛋白和前清蛋白含量高于EN组,差异均有显著性(F=4.12～26.71,P<0.05)。结论高血压脑出血术后病人早期给予PN+EN支持可提高病人免疫功能。
Objective To study the effect of parenteral nutrition（PN）,enteral nutrition（EN） and PN plus EN on humoral immune function（HIF） in patients after surgery for hypertensive cerebral hemorrhage（HCH）.Methods Ninety patients after surgery for HCH were evenly randomized to PN,EN and PN＋EN group,a nutritional support was offered accordingly.On the first and 14th day of admission,peripheral venous blood was collected for detection of IgA,IgG,IgM,albumin and prealbumin,the changes of humoral immunity were compared among the three groups.Results On the first day of admission,the differences of each item detected among the three groups were not significant.As compared between the first day and 14th day of admission;in PN group,the difference among each item detected was not significant;in EN group,the levels of IgA,IgG and albumin elevated;in PN＋EN group,all the items detected increased,the differences being significant（t=2.16-3.01,P〈0.05）.On the 14th day of admission,the levels of IgM,albumin and prealbumin in EN group were higher than that in PN;the each item detected inPN＋EN group was higher than that in PN,and the levels of IgA,albumin and prealbumin were higher than that in EN,the differences being significant（F=4.12-26.71,P〈0.05）.Conclusion In patients after surgery for HCH,an earlyPN＋EN support is the best choice to raise the patient＇s immune function.
