改良臭氧注射法及PLD治疗腰椎间盘突出症的对照研究 被引量:3

摘要 目的对比观察改良臭氧注射法及经皮腰椎间盘切吸术治疗腰椎间盘突出症的疗效、并发症,探讨具体治疗的安全性。方法将120例腰椎间盘突出症患者随机分成两组,应用改良臭氧注射法治疗的为A组(60例),应用经皮腰椎间盘切吸术治疗的为B组(60例),对两组患者术后1周和6个月疗效、穿刺治疗时间、住院时间及并发症进行统计学分析。结果优良率在1周时A、B组分别为55%和38.3%,差异有统计学意义(P<0.05);在6个月时,A、B组分别为68.3%和70%,差异无统计学意义(P>0.05);并发症发生率A、B组分别为13.3%、33.3%,B组明显高于A组(P<0.01);A组穿刺治疗时间(18.0±3.7)m in,B组为(42.2±6.5)m in;A组住院时间(4.7±0.6)d,B组为(6.0±0.9)d,两组穿刺治疗时间及住院时间比较均有显著性差异(P<0.01)。结论 O3注射治疗及经皮腰椎间盘切吸术具有相似疗效,且均具有微创、并发症较少的优点。而改良的O3注射法由于并发症更少,操作更简单,住院时间更短,效价比更有优势,因此更适宜基层医院开展。 Objective To compare the therapeutic effects and complications of modified ozone injection and percutaneous lumbar disketomy(PLD) in treatment of lumbar disc herniation(LDH),and to discuss safety of the therapy.Methods 120 cases of LDH were randomly divided into two groups.Group A was treated with modified injection of ozone,group B was treated with percutaneous lumbar disketomy.The therapeutic result,operative time,day in hospital and complications were compared between the two groups at 1 week and 6 months post-operation.Results There was a significant difference in good rates between group A and B after a week(55% and 38.3% respectively),however,this difference disappeared at six months after treatment.The group A has shorter operative time(18.0±3.7 min for group A,42.2±6.5 min for group B),quicker recovery,shorter hospital stay(4.7±0.6 d for group A and 6.0±0.9 d for group B),and less postoperative complications(13.3% group A and 33.3% group B).Conclusion The curative effect of modified ozone injection in treatment of LDH is similar to PLD and both of them have the advantages of less invasion,but the modified ozone injection have less complication,easier operation,shorter hospital stays and lower cost than PLD,the modified ozone injection is more suitable to basic hospitals.
