摘要
目的探讨后腹腔镜下肾蒂淋巴管剥脱术治疗乳糜尿的临床疗效。方法收集我院2008年1月至2010年9月35例乳糜尿病例,均采用经后腹腔镜肾蒂淋巴管剥脱术治疗。结果 35例手术均顺利完成,无中转开放,平均手术时间75min;术中平均出血量42mL;术后平均住院7d,术后肠功能恢复时间1～2d。术中术后无明显外科并发症。术后恢复顺利,平均7d出院。34例尿液清亮、乳糜尿试验阴性,1例术后第3天出现尿液轻微混浊,乳糜试验弱阳性,给予2%硝酸银肾盂灌注,尿液转为清亮。术后随访6～24个月,35例患者症状均未复发。结论后腹腔镜肾蒂淋巴管结扎术手术疗效肯定,术中出血少,术后恢复快,对患者损伤小,并发症少,是一种安全、有效的治疗方法。
Objective To explore the clinical efficacy of renal lymphatic stripping with retroperitoneoscopy.Methods Clinical data of 35 cases of chyluria treated with retroperitoneoscopic renal lymphatic stripping were retrospectively analyzed.Results The operations were successful in all patients.The mean operation time was 75 minutes,blood loss 42mL,hospitalization 7 days,recovery time of gastrointestinal function 1-2 days.No surgical complications occurred.The urine was clear and the chylous tests were negative in 34 patients.One patient showed chyluria the third day after operation.After pelvis perfusion of 2% silver nitrate,the urine turned clear.No recurrence of chyluria was found in all the 35 cases during the follow-up of 6-24 months.Conclusions Retroperitoneoscopic stripping of renal lymphatic vessels was effective and efficient in the management of chyluria,with advantages of less trauma,less blood loss,fewer complications,and rapid recovery.
